肺癌患者家庭回避癌症沟通现状调查及影响因素分析

摘　　要：目的 了解肺癌患者家庭回避癌症沟通现状及影响因素。方法 采用便利抽样法,选择2022年5月-8月住院治疗的186例肺癌患者作为研究对象,采用一般资料调查表、家庭回避癌症沟通量表、疾病不确定感量表以及恐惧疾病进展简化量表对患者进行横断面调查。结果 肺癌患者家庭回避癌症沟通量表得分为(42.58±11.89)分,多元线性回归分析结果显示,肺癌患者性别、癌症分期、疾病不确定感以及恐惧疾病进展水平为其家庭回避癌症沟通的独立影响因素(F=13.612,P<0.001),可解释总变异的68.9%。结论 肺癌患者家庭回避癌症沟通处于中等水平,其中男性、癌症晚期、疾病不确定感水平较低、恐惧疾病进展水平越高的肺癌患者更应引起医护人员的重视,采用个体化的干预方案,以降低肺癌患者家庭回避癌症沟通水平。Objective To understand the status quo of avoiding cancer communication in the family of patients with lung cancer and analyze its influencing factors.Methods Using convenience sampling method,186 lung cancer patients hospitalized between May and August 2022 were selected as the study subjects.They were given a cross-sectional survey using a general condition questionnaire,the family avoidance of communication about cancer scale(FACC),the Mishel uncertainty in illness scale(MUIS)and the fear of progression questionnaire-short form(FoP-Q-SF).Results The score of FACC was 42.58±11.89.According to the results of multiple linear regression analysis,the gender,cancer staging,disease uncertainty and fear of disease progression were the independent factors affecting their family avoiding cancer communication(F=13.612,P<0.001),which could explain 68.9%of the total variation.Conclusion The family avoidance of cancer communication of lung cancer patients is at a medium level.Male patients,with advanced cancer,lower level of disease uncertainty and higher level of fear of disease progression should be paid more attention to by medical personnel,and individualized intervention programs should be provided to reduce the level of family avoidance of cancer communication of lung cancer patients.
