摘 要:目的系统评价能力本位教育(CBE)在护士培训中运用效果,为护士实施CBE提供参考。方法检索国内外相关数据库,收集CBE对护士培训的随机对照试验类研究(RCT),使用Review Manager 5.3软件对研究结果进行Meta分析。结果本研究共纳入9篇文献。Meta分析结果显示,CBE可提高护士的理论知识[SMD=1.56,95%CI(1.03~2.09),P<0.001]、操作技能[SMD=1.04,95%CI(0.69~1.38),P<0.001]、领导能力[SMD=0.65,95%CI(0.07~1.24),P=0.03]、专业建设与发展能力[SMD=1.42,95%CI(0.33~2.51),P=0.01]、批判性思维能力[SMD=0.59,95%CI(0.39~0.78),P<0.001]、临床护理能力[SMD=0.97,95%CI(0.31~1.64),P=0.004]、人际沟通能力[SMD=0.74,95%CI(0.03~1.45),P=0.04],而对于教育与咨询能力[SMD=0.67,95%CI(-0.18~1.53),P=0.12]和法律伦理与实践能力[SMD=0.93,95%CI(-0.51~2.38),P=0.21],传统培训方法和CBE的培训效果比较差异无统计学意义。结论CBE可有效提高护士的理论知识、操作技能、领导能力、专业建设与发展能力、批判性思维能力、临床护理能力、人际沟通能力,该培训方式值得护理管理者借鉴,但需扩大样本量进行研究验证。Objective To systematically evaluate the application eff ect of competency based education(CBE)in nurse training,and provide reference for nurses to implement CBE.Methods Relevant databases at home and abroad were searched to collect randomized controlled trials(RCT)of CBE on nurse training,and the results were Meta-analysis by using the Review Manager 5.3 software.Results A total of 9 articles were included in this study.The Meta-analysis results showed that the CBE can improve nurses'theoretical knowledge[SMD=1.56,95%CI(1.03~2.09),P<0.001],operational skills[SMD=1.04,95%CI(0.69~1.38),P<0.001],leadership ability[SMD=0.65,95%CI(0.07~1.24),P=0.03],professional construction and development ability[SMD=1.42,95%CI(0.33~2.51),P=0.01],critical thinking ability[SMD=0.59,95%CI(0.39~0.78),P<0.001],clinical nursing ability[SMD=0.97,95%CI(0.31~1.64),P=0.004],and interpersonal communication ability[SMD=0.74,95%CI(0.03~1.45),P=0.04],while for education and consulting ability[SMD=0.67,95%CI(-0.18~1.53),P=0.12],legal ethics and practical ability[SMD=0.93,95%CI(-0.51~2.38),P=0.21],there was no statistical difference between traditional training methods and CBE training eff ect.Conclusion CBE can eff ectively improve nurses'theoretical knowledge,operational skills,leadership ability,professional construction and development ability,critical thinking ability,clinical nursing ability,and interpersonal communication ability,this training method was worth learning from nursing managers,but needs to expand the sample size for research and verification.
