基于数据挖掘探讨国医大师周学文教授治疗胃脘痛用药规律

摘　　要：目的基于中医传承计算平台(V3.0)软件,对国医大师周学文教授治疗胃脘痛的用药经验进行回顾性研究,传承其学术思想。方法将所收集的112例胃脘痛首诊医案运用频次统计、关联规则、聚类分析等数据挖掘方法,探讨周老用药规律和辨治经验。结果112张处方的性别统计中以女性最多,四诊统计中以胃脘痛、胃胀、食欲不振、反酸,舌质红、苔黄厚,脉弦滑为主;涉及用药84味,其中频次排前7味的有浙贝母、黄芪、白豆蔻、海螵蛸、砂仁、苦参、鸡内金。药物以清热类最多,其次为补虚类、化湿类;四气统计中以寒性药最多,其次是温性药;五味统计中最多的是苦味药,其次是甘味药、辛味药;药物以归入脾经、胃经、肝经最多;将支持度设置为50,置信度设置为0.95,得到核心药物组合共24种;根据聚类分析(k-means算法与回归模型相结合)得到新方组合6类。结论国医大师周学文教授治疗胃脘痛重视调畅全身气机,以清热泻火解毒、燥湿和胃止痛、祛瘀敛疮生肌为治疗法则,以期为今后诊治胃脘痛提供一定参考。Objective Based on the software of the TCM Inheritance Computing Platform(V3.0),a retrospective study was conducted on the medication experience of Professor ZHOU Xuewen,a master of traditional Chinese medicine,in the treatment of epigastric pain,and his academic ideas were inherited.Methods The first medical cases of 112 cases of epigastric pain were collected,using data mining methods such as frequency statistics,correlation rules,and cluster analysis,the rules of Professor ZHOU medication and the experience of defense and treatment were discussed.Results Among the 112 prescription sex statistics,women were the most,and in the four diagnostic statistics,epigastric pain,stomach bloating,loss of appetite,acid reflux,red tongue,thick moss,and smooth pulse strings were the mainstays;84 flavors were involved,of which the top 7 flavors were Zhebeimu(Fritillariae Thunbergii Bulbus),Huangqi(Astragali Radix),Baidoukou(Amomi Fructus Rotundus),Sharen(Amomi Fructus),Kushen(Sophorae Flavescentis Radix),Jineijin(Galli Gigerii Endothelium Corneum).The heat-clearing drugs were the most,followed by the tonic and wet categories;among the four Qi statistics,the cold drugs were the most,followed by the warm drugs;the most in the five-taste statistics were bitter medicines,followed by sweet medicines and spicy medicines;the drugs were classified into the spleen meridian,stomach meridian,and liver meridians;the support degree was set to 50,the confidence degree was set to 0.95,and a total of 24 core drug combinations were obtained;According to cluster analysis(k-means algorithm combined with regression model),the new square combination of 6 classes was obtained.Conclusion Professor ZHOU Xuewen,a master of traditional Chinese medicine,attaches great importance to the treatment of epigastric pain,and attaches importance to the smoothing of the whole body Qi machine,so as to clear the heat and relieve the fire,dryness and dampness and stomach pain,and the removal of bruising sores and muscles as the treatment rules,in order to provide
