摘　　要：目的 探讨食管小细胞癌(small cell esophageal carcinoma,SCEC)手术切除患者SRY相关高迁移率族盒蛋白2(SRY related high-mobility-group box protein 2,SOX2)的表达及其预后价值。方法 通过免疫组织化学方法检测SCEC患者癌组织中SOX2的表达,同时检测癌周正常食管黏膜中SOX2的表达,采用Spearman相关性分析SOX2表达水平与临床病理参数的关系,Kaplan-Meier生存分析和COX比例风险模型分析影响SCEC预后的危险因素。结果 118例SCEC患者中SOX2的阳性率为46.6%(55/118),其中高表达组27.1%(32/118),低表达组19.5%(23/118),SOX2的表达与局部浸润深度、TNM分期、淋巴结转移、远处转移相关;SOX2高表达组更多见于TNMⅢ~Ⅳ期、局部浸润纤维膜、有淋巴结转移和远处转移的患者;SOX2低表达组更多见于肿瘤直径<2 cm和2~4 cm、局部浸润黏膜下层和肌层的患者。Kaplan-Meier生存分析结果显示肿瘤直径、TNM分期、局部浸润深度、淋巴结转移、远处转移、SOX2高表达与患者预后有关(P<0.05),COX比例风险模型分析结果显示,TNM分期、SOX2高表达、远处转移是患者预后的独立影响因素。结论 SOX2高表达可作为SCEC患者术后预后不良的指标,SOX2有望成为SCEC的预后生物标志物。Objective To investigate the expression and prognostic value of SRY related high-mobility-group box protein 2(SOX2)in patients with small cell esophageal carcinoma(SCEC).Methods The expression of SOX2 in cancer tissues of SCEC patients was detected by immunohistochemical method,and the expression of SOX2 in normal esophageal mucosa around the cancer was detected.The relationship between SOX2 expression level and clinicopathological parameters was analyzed by Spearman correlation,and Kaplan-Meier survival analysis and COX proportional hazard model were used to analyze the risk factors affecting the prognosis of SCEC.Results The positive rate of SOX2 in 118 patients with SCEC was 46.6%(55/118)including 27.1%(32/118)in the high expression group and 19.5%(23/118)in the low expression group.The expression of SOX2 was related to the depth of local invasion,TNM stage,lymph node metastasis and distant metastasis.SOX2 high expression was more common in patients with TNM stageⅢ-Ⅳ,local infiltration of fibrous membrane,lymph node metastasis and distant metastasis,while SOX2 low expression was more common in patients with tumor diameter<2 cm and 2-4 cm,and local infiltration of submucosa and muscularis.Kaplan Meier survival analysis showed that tumor diameter,TNM stage,depth of local invasion,lymph node metastasis,distant metastasis,and high expression of SOX2 were related to the prognosis of patients(P<0.05).COX proportional hazard model analysis showed that TNM stage,high expression of SOX2,and distant metastasis were independent factors affecting the prognosis of patients.Conclusion The high expression of SOX2 can be used as an indicator of poor prognosis in patients with SCEC,and SOX2 is expected to become a prognostic biomarker of SCEC.
