摘  要:目的:探讨急性缺血性脑卒中早期NOD样受体热蛋白结构域相关蛋白3(NLRP3)炎症小体的变化特征。方法:纳入98例急性缺血性脑卒中患者,并按神经功能缺损的严重程度分为轻度、中度、重度三组,采集血液样本,采用RT-qPCR分析血液中NLRP3 mRNA表达水平;分析其与神经功能缺失及脑梗死体积的关系。结果:三组中罹患冠心病、高血压、高血脂和高血糖的比例,差异有统计学意义(P<0.05);年龄、性别、吸烟和饮酒方面,三组间差异无统计学意义(P>0.05)。与轻度神经功能缺损组(轻度组)相比,在发病6、12和24 h,中、重度组的NLRP3 mRNA表达水平显著增高(P<0.05)。与小梗死组比较,中、大梗死组NLRP3 mRNA表达水平显著增高(P<0.05)。结论:血液中NLRP3表达水平越高,急性缺血性脑卒中患者神经功能缺损评分越高,脑梗死体积增大。NLRP3表达水平与梗死损伤的严重程度相关。Objective: To investigate the change of NOD-like receptor pyrin domain containing 3(NLRP3) inflammasome in acute ischemic stroke and its association with neurological deficits. Methods: The clinical data of 98 cases with acute ischemic stroke were included, and the patients were divided into three group according to the severity of neurological deficit. Blood samples were collected at the end of 6, 12 and 24 hours after stroke onset, and the mRNA expression of NLRP3 in blood was quantitatively analyzed by RT qPCR. The relationship between neurological deficit and infarction volume was analyzed. Results: Among the three groups, significant difference appeared in the history of coronary heart disease, hypertension, hyperlipidemia, and hyperglycemia(P<0.05), while there was no significant difference in age, gender, smoking and drinking(P>0.05). Compared with that respectively in mild neurological deficit group, the mRNA expression of NLRP3 at 6, 12, and 24 h was significantly higher in either moderate or severe neurological deficit group(P<0.05). Compared with that in small infarction group, the mRNA expression of NLRP3 in medium and large infarction groups was significantly higher(P<0.05). Conclusion: Higher transcription level of NLRP3 in blood was associated with elevated neurological deficits and enlarged cerebral infarction. NLRP3 may reflect the severity of ischemic stroke.
