衍生的中性粒细胞淋巴细胞比值和肺免疫预后指数对332例非手术老年非小细胞肺癌患者预后的影响分析

摘　　要：[目的]探讨衍生的中性粒细胞淋巴细胞比值(dNLR)和肺免疫预后指数(LIPI)对非手术老年非小细胞肺癌患者预后的影响。[方法]回顾性分析332例非手术老年非小细胞肺癌患者的临床数据,分析患者治疗后5年内生存情况的影响因素。比较不同dNLR水平及不同LIPI水平患者治疗后5年内的生存情况。[结果]患者治疗前dNLR为2.49±1.31,治疗前LIPI评分:良好135例,中等162例,差35例。多因素分析结果显示:患者年龄、肿瘤分化程度、临床分期、dNLR、化疗情况、放疗情况、LIPI均为预后影响因素(P<0.05)。低dNLR水平组患者的中位生存时间为41.580个月,高dNLR水平组为33.330个月,差异有统计学意义(χ2=13.035,P<0.05)。LIPI水平良好组、中等组、差组患者中位生存时间分别为51.340个月、29.590个月和8.580个月,差异有统计学意义(χ2=373.576,P<0.05)。[结论]治疗前的dNLR以及LIPI可影响非手术老年非小细胞肺癌患者的预后,对患者的预后预测有一定价值。[Objective]To explore the application of derived neutrophil lymphocyte ratio(dNLR)and lung immune prognostic index(LIPI)in the prognosis of non-small cell lung cancer(NSCLC)for elderly non-surgical patients.[Methods]The clinical data of 332 elderly non-surgical NSCLC patients were retrospectively analyzed.The relationship of survival of patients with dNLR,LIPI and other indicators was analyzed with multivariate regression analysis.[Results]Before treatment,the mean dNLR of patients was 2.49±1.31;LIPI scores showed the good prognosis in 135 cases,moderate in 162 cases,and poor in 35 cases.Multivariate analysis showed that gender,tumor differentiation,clinical stage,dNLR,chemotherapy,radiotherapy and LIPI were all prognostic factors(P<0.05).The median survival time of patients with low dNLR level and high dNLR level was 41.580 months and 33.330 months,respectively(χ2=13.035,P<0.05).The median survival time of patients with good,medium and poor LIPI level was 51.340 months,29.590 months and 8.580 months respectively(χ2=373.576,P<0.05).[Conclusion]The dNLR and LIPI before treatment can predict the prognosis of elderly non-surgical NSCLC patients.
