摘  要:目的探讨动脉自旋标记(ASL)静音磁共振血管成像(MRA)在脑动静脉畸形(AVM)中的应用价值.方法选取经临床诊断疑似脑AVM患者34例,均行静音MRA及三维时间飞跃法磁共振血管成像(3D TOF MRA)检查,在MRA检查后1周内行数字减影血管造影(DSA)检查.以DSA结果为金标准,计算2种MRA技术对脑AVM供血动脉、引流静脉的诊断效能,并采用配对χ^(2)检验比较2种MRA技术对供血动脉、瘤巢及引流静脉显示情况的差异.结果34例患者中,DSA诊断脑AVM 30例,其中DSA共检出瘤巢30个,供血动脉44根,引流静脉33根;4例为颅内动脉狭窄(其中右侧大脑中动脉M1段狭窄3例,左侧大脑后动脉P1段狭窄1例).2种MRA技术对供血动脉、瘤巢显示无统计学意义(P值分别为0.125,0.250),而2种MRA技术对引流静脉显示有统计学意义(P=0.002).结论静音MRA能准确、可靠地评估脑AVM的瘤巢、供血动脉及引流静脉,尤其在引流静脉方面明显优于3D TOF MRA,可为脑AVM提供更全面的诊断信息.Objective To investigate the application value of silent magnetic resonance angiography(MRA)based on arterial spin labeling(ASL)in cerebral arteriovenous malformation(AVM).Methods A total of 34 patients with suspected cerebral AVM were selected,silent MRA and three dimensional time of flight magnetic resonance angiography(3D TOF MRA)were performed,and digital subtraction angiography(DSA)was performed within one week after MRA.Taking the DSA results as the gold standard,the diagnostic efficacy of the two MRA techniques for the blood supply arteries and drainage veins of cerebral AVM was calculated,and the differences in the display of the blood supply arteries,tumor nests and drainage veins between the two MRA techniques were compared by paired chi-square test.Results Among the 34 patients,30 cases of cerebral AVM were diagnosed by DSA,including 30 tumor nests,44 blood supply arteries,33 drainage veins,and 4 cases of intracranial artery stenosis(including 3 cases of M1 seg-ment stenosis of the right middle cerebral artery and 1 case of P1 segment stenosis of the left posterior cerebral artery).The two MRA techniques had no statistical significance in the display of blood supply arteries and tumor nests(P=0.125 and 0.250,respectively),while the two MRA techniques had statistical significance in the display of drainage veins(P=0.002).Conclusion Silent MRA can accurately and reliably evaluate the tumor nests,blood supply arteries and drainage veins of cerebral AVM,especially in drainage veins,which is significantly better than 3D TOF MRA.Therefore,it can provide more comprehensive diagnostic information for cerebral AVM.
