摘  要:目的分析同时性结直肠癌肝转移灶的MRI影像组学特征,构建Logistic模型预测化疗疗效。方法收集经病理证实102例结直肠癌患者的223个同时性肝转移灶,分析T_(2)WI、扩散加权成像(DWI)及门脉期图像。勾画肝转移灶感兴趣区后,计算其影像组学特征。以7︰3的比例随机将数据分成训练集和验证集,经特征筛选后构建预测肝转移灶化疗疗效的影像组学模型,包括基于T_(2)WI(LR-T_(2)WI)、DWI(LR-DWI)、门脉期(LR-门脉期)及结合3个MRI序列(LR-MRI)的Logistic模型。计算其曲线下面积(AUC)值评估模型效能。结果单独MRI序列的LR-DWI鉴别效能高于LR-T_(2)WI及LR-门脉期,其训练集AUC为0.652[95%置信区间(CI)0.571~0.726],验证集AUC为0.661(95%CI 0.536~0.772)。3个序列的LR-MRI鉴别效能较好,训练集AUC为0.733(95%CI 0.656~0.800),验证集AUC为0.753(95%CI 0.633~0.849)。结论基于MRI的T_(2)WI、DWI及门脉期的影像组学分析有助于预测同时性结直肠癌肝转移灶的化疗疗效。Objective To develop Logistic and MRI-based radiomics models to predict the response of chemotherapy in synchronous liver metastasis of colorectal carcinoma.Methods A total of 223 synchronous liver metastasis lesions from 102 patients of colorectal carcinoma confirmed by pathology were collected.The T_(2)WI,diffusion weighted imaging(DWI),and portal phase of MRI images were analyzed.After the segmentation of region of interest,the radiomic features were automatically calculated.Then the dataset were divided into the training cohort and the testing cohort with a proportion of 7︰3,randomly.After feature selection,a radiomics model was developed to predict the response of chemotherapy for liver metastasis,the Logistic models of T_(2)WI(LR-T_(2)WI),DWI(LR-DWI),portal phase(LR-portal phase),and the combined model(LR-MRI)were developed.The corresponding area under the curve(AUC)value were calculated to evaluate the efficiency of models.Results To the separated sequence,the AUC of LR-DWI were higher than those of LR-T_(2)WI and LR-portal phase with AUC of 0.652[95%confidence interval(CI)0.571-0.726]in the training cohort and 0.661(95%CI 0.536-0.772)in the testing cohort.The LR-MRI could better predict the response of chemotherapy with AUC of 0.733(95%CI 0.656-0.800)in the training cohort and 0.753(95%CI 0.633-0.849)in the testing cohort.Conclusion MRI-based radiomics signature with T_(2)WI,DWI,and portal phase sequences can help predict the response of chemotherapy in synchronous liver metastasis of colorectal carcinoma.
