卵巢肿瘤蒂扭转的MRI表现及病理对照分析

摘　　要：目的结合病理对照分析卵巢肿瘤蒂扭转的MRI特征表现。方法回顾性分析21例经手术病理确诊为卵巢肿瘤蒂扭转患者的病理及MRI资料,所有患者均行MRI平扫和动态增强检查。结果21例卵巢肿瘤均为良性,其中9例位于左侧,12例位于右侧。MRI诊断符合率66.7%(14/21),扭转的蒂部呈“绳索状”“管带状”5例,“漩涡征”4例,肿瘤边缘团片状异常信号3例,肿瘤外周出现“囊泡征”2例。9例伴出血性梗死患者中7例肿瘤壁增厚、不强化,8例扩散加权成像(DWI)呈高信号、表观扩散系数(ADC)呈低信号。结论卵巢肿瘤蒂扭转在MRI上会出现“旋涡征”,边缘见团片状异常信号等特征性表现,肿瘤壁增厚不强化、DWI高信号并ADC低信号具有提示出血梗死的意义。Objective To compare the MRI features and pathological findings in patient with ovarian tumor pedicle torsion.Methods The pathological and MRI data of 21 patients confirmed by surgical pathology as ovarian tumor pedicle torsion were analyzed retrospectively,and all patients underwent MRI plain and dynamic enhancement examinations.Results All ovarian tumors(n=21)were benign,with 9 cases on the left and 12 cases on the right.The MRI diagnostic accordance rate was 66.7%(14/21).There were 5 cases with“rope-like”and“tubular band-like”twisted pedicle,4 cases with“vortex sign”,3 cases with flaky-like abnormal signal at the edge of the tumor,and 2 cases with“vesicle sign”at tumor periphery.Of 9 patients with hemorrhagic infarction,7 cases showed thickened and non-enhancing tumor walls and 8 cases presented high signal on diffusion weighted imaging(DWI)and low signal on apparent diffusion coefficient(ADC).Conclusion Ovarian tumor pedicle torsion show the characteristic manifestations such as“vortex sign”and flaky-like abnormal signal at the edge on MRI,and tumor wall thickening without enhancement,high signal on DWI and low signal on ADC have the significance of suggesting hemorrhagic infarction.
