摘 要:目的探讨MRI和临床参数在前列腺癌精囊腺浸润(SVI)中的预测价值。方法回顾性分析经病理证实的前列腺癌患者119例,收集患者的临床特征及参数,包括年龄、体质量指数(BMI)、前列腺特异性抗原(PSA)、Gleason评分(GS),以及MRI前列腺影像报告和数据系统第2.1版(PI-RADS v2.1)信息。对所有临床和影像学变量进行单因素和多因素Logistic回归分析和受试者工作特征(ROC)曲线分析,并绘制MRI参数预测模型、Logistic回归分析得到联合SVI预测模型的ROC曲线。结果单因素分析显示PSA、GS(≥8分)、MRI均与SVI显著相关(P<0.05)。在临床参数的多因素Logistic回归分析中,SVI预测因子为PSA(P=0.024)及GS(≥8分)(P=0.013)。在临床及MRI多因素Logistic回归分析中,MRI预测SVI的P=0.002,PSA预测SVI的P=0.016,GS(≥8分)预测SVI的P=0.026。MRI、Logistic回归分析得到的MRI联合临床参数预测概率模型曲线下面积(AUC)分别为0.662、0.860。结论MRI结合临床参数对预测前列腺癌患者SVI有显著统计学意义,且AUC值更高,具有较高的预测价值,为临床医生制订治疗方案提供了依据和证据。Objective To investigate the value of MRI and clinical parameters in predicting seminal vesicle invasion(SVI)of pros-tate cancer.Methods A total of 119 pathological prostate cancer patients were analyzed retrospectively.The clinical characteristics and parameters of the patients were collected,including age,body mass index(BMI),prostate specific antigen(PSA),Gleason score(GS),and MRI prostate imaging reporting and data system version 2.1(PI-RADS v2.1).Univariate and multivariate Logistic regression analysis and receiver operating characteristic(ROC)curve analysis were performed on all clinical and imaging variables,and the ROC curve of the combined SVI prediction model was obtained by combining the MRI parameter prediction model and Logistic regression analysis.Results Univariate analysis showed that PSA,GS(≥8 points)and MRI were significantly correlated with SVI(P<0.05).In the multivariate Logistic regression analysis of clinical parameters,the predictors of SVI were PSA(P=0.024)and GS(≥8 points)(P=0.013).In clinical and MRI multivariate Logistic regression analysis,P value of MRI and PSA,GS(≥8 points)in predicting SVI were 0.002,0.016 and 0.026.The area under the curve(AUC)of MRI combined with clinical parameters predicted by MRI and Logistic regression analysis were 0.662 and 0.860 respectively.Conclusion MRI combined with clinical parameters has statistical significance in predicting SVI in patients with prostate cancer,and AUC value is higher,which has high predictive value,providing basis and evidence for clinicians to make therapeutic schedule.
