Rac1与Cdc42表达对鼻咽癌恶性表型及预后的影响

摘　　要：目的探讨Rac1和Cdc42表达情况对鼻咽癌恶性表型及预后的影响。方法选取2018年2月至2020年8月在自贡市第三人民医院就诊的98例鼻咽癌患者作为观察组,其中II期患者24例,Ⅲ期患者56例,Ⅳa/b期患者18例。另外选取同一时期来我院体检的鼻咽部上皮组织正常的30例体检人员作为对照组。分别获取观察组和对照组研究对象的鼻咽部活检组织标本,采用过氧化物酶(streptavidin-peroxidase,SP)免疫组化方法对组织标本进行免疫组化检测。观察组所有患者均给予根治性放射治疗,II期患者给予6MV-X线分割照射,Ⅲ期和Ⅳa/b期患者给予同期含铂方案单药联合化疗,探讨Racl、Cdc42表达与鼻咽癌恶性表型及预后判断的相关性。结果免疫组化切片染色结果显示,观察组患者鼻咽癌组织Rac1、Cdc42均呈阳性反应,对照组鼻咽部正常组织Rac1、Cdc42均呈阴性表达。观察组鼻咽癌组织中Rac1、Cdc42阳性率均显著高于对照组,差异具有统计学意义(χ^(2)值分别为48.82、26.67,P值均<0.05)。观察组鼻咽癌组织中Rac1、Cdc42的表达水平在不同临床分期中差异明显,TNM分期越高,Rac1和Cdc42阳性表达率越高,组间差异具有统计学意(Rac1:r_(T,N,Group)=0.42,0.32,0.39,P值均<0.05;Cdc42:r_(T,N,Group)=0.39,0.38,0.44,P值均<0.05)。鼻咽癌组织中Rac1和Cdc42表达呈显著正相关(r=0.37,P<0.05)。结论鼻咽癌患者鼻咽癌组织中Rac1、Cdc42表达均呈阳性,Rac1和Cdc42表达水平与鼻咽癌病情和TNM分期相关,TNM分期越高,Rac1和Cdc42表达率越高。Objective To investigate the influence of Rac1 and Cdc42 expression on the malignant phenotype and prognosis of nasopharyngeal carcinoma.Methods Total of 98 patients with nasopharyngeal carcinoma who were treated in Zigong Third People's Hospital from August 2012 to February 2010 were selected as the observation group,with 24 patients in stage II,56 patients in stageⅢ,and 18 patients in stageⅣa/b.In addition,30 physical examiners with normal nasopharyngeal epithelium who came to our hospital for physical examination in the same period were selected as the control group.The nasopharyngeal biopsy specimens were obtained from the observation group and control group respectively.The streptavidin-peroxidase(SP)was used to detect the tissue samples.All patients in the observation group were treated with radical treatment.Phase II patients were given 6MV-X ray fractionation irradiation,and phaseⅢandⅣa/b patients were given single drug combined chemotherapy containing platinum at the same time.The correlation between the expression of Rac1,Cdc42 and the malignant phenotype,prognosis of nasopharyngeal carcinoma were explored.Results The immunohistochemical staining results showed that Rac1 and Cdc42 were positive in nasopharyngeal carcinoma tissues of in the observation group,while Rac1 and Cdc42 were negative in normal nasopharyngeal tissues of the control group.The positive rates of Rac1 and Cdc42 in nasopharyngeal carcinoma tissues in the observation group were significantly higher than those in the control group(χ^(2) values were 48.82 and 26.67 respectively,both P values<0.05).The expression levels of Rac1 and Cdc42 in nasopharyngeal carcinoma tissues in the observation group were significantly different in different clinical stages.The higher the TNM stage,the higher the positive expression rate of Rac1 and Cdc42(Rac1:r_(T,N,Group)=0.42,0.32,0.39,all P values<0.05;Cdc42:r_(T,N,Group)=0.39,0.38,0.44,all P values<0.05).The expression of Rac1 and Cdc42 in nasopharyngeal carcinoma was significantly positively c
