摘 要:目的探究磁共振(MR)弥散加权成像(DWI)联合动态增强曲线对乳腺肿瘤良恶性鉴别诊断价值。方法回顾性分析2021年8月-2022年8月广东医科大学附属医院收治的86例乳腺肿瘤患者临床资料,所有患者均于入院时接受常规磁共振成像(MRI)平扫、DWI、动态增强扫描,并接受病理检查。比较乳腺良性肿瘤、恶性肿瘤患者的表观扩散系数(ADC)值、早期强化率,分析二者联合诊断鉴别乳腺肿瘤良恶性的价值。结果86例患者经病理检查确诊59例为良性肿瘤,27例为恶性肿瘤。乳腺良性、恶性肿瘤患者的TIC曲线分型比较,差异有统计学意义(P<0.05)。乳腺恶性肿瘤患者的ADC值低于乳腺良性肿瘤患者,早期强化率高于乳腺良性肿瘤患者(P<0.05)。绘制ROC曲线图结果显示,ADC值、早期强化率及联合诊断乳腺肿瘤良恶性的AUC为0.883、0.842、0.904,均有一定诊断价值,且当ADC值、早期强化率取0.995×10^(-3)mm^(2)/s、88.79%时,可获得最佳诊断效能,联合诊断价值较高。结论乳腺良恶性肿瘤患者的ADC值、动态增强曲线存在明显差异,ADC值联合早期强化率可有效鉴别诊断乳腺肿瘤良恶性,诊断价值较高。Objective To explore the value of MR Diffusion-weighted imaging(DWI)combined with dynamic enhancement curve in the differential diagnosis of benign and malignant breast tumors.Methods Clinical data of 86 patients with breast tumor admitted to our hospital from August 2021 to August 2022 were retrospectively analyzed.All patients received conventional magnetic resonance imaging(MRI)plain scan,DWI scan,dynamic enhancement scan,and pathological examination on admission.The value of apparent diffusion coefficient(ADC)and early enhancement rate in patients with benign and malignant breast tumors were compared to analyze the value of their combined diagnosis in differentiating benign and malignant breast tumors.Results 59 cases of benign tumor and 27 cases of malignant tumor were confirmed by pathological examination.There was statistical significance in the classification of TIC curves between benign and malignant breast tumors(P<0.05).The ADC value of patients with breast malignant tumor was lower than that of patients with breast benign tumor,and the early enhancement rate was higher than that of patients with breast benign tumor(P<0.05).ROC curve drawing results showed that ADC value,early enhancement rate and AUC of combined diagnosis of benign and malignant breast tumors were 0.883,0.842 and 0.904,all of which had certain diagnostic value.The optimal diagnostic efficiency could be achieved when ADC value and early enhancement rate were 0.995×10^(-3)mm^(2)/s and 88.79%.Combined diagnosis value is higher.Conclusion There are significant diferences in ADC value and dynamic enhancement curve in patients with benign and malignant breast tumors.ADC value combined with early enhancement rate can effectively differentiate benign and malignant breast tumors with high diagnostic value.
