PSP术与SP术治疗腮腺浅叶良性肿瘤的效果对比分析

摘　　要：目的腮腺浅表良性肿瘤患者,经过腮腺浅表部分切除术(PSP)和腮腺浅表切除术(SP)治疗效果对比分析。方法回顾性分析郑州大学附属洛阳中心医院2016年5月至2021年5月收治的68例腮腺浅叶良性肿瘤患者,按照治疗方法的不同分为两组,其中PSP术34例,SP术34例,观察两组各项手术指标、手术前后相关评分[体像担忧问卷(BICI)、身体意象量表(BIS)、医院焦虑抑郁量表(HADS)]、术后并发症情况、生活质量量表(SF-36)评分。结果术前两组BICI、BIS、HADS、SF-36评分差异无统计学意义(P>0.05);PSP组术中失血量、解剖面神经分支数少于SP组,手术时间、切口长度短于SP组;出院前两组评分均下降,其中PSP组BICI、BIS、HADS焦虑、HADS抑郁评分均低于SP组;PSP组术后并发症率发生率(14.71%)低于SP组(35.29%);出院前PSP组SF-36评分高于SP组,差异有统计学意义(P<0.05)。结论腮腺浅叶部分切除术对腮腺浅叶良性肿瘤的治疗可有效缩短手术时间,减少术中出血量,改善患者负面情绪,并降低各项并发症发生情况,提升患者术后生活质量,整体疗效理想。Objective To compare and analyze the therapeutic effects of superficial partial parotidectomy(PSP)and superficial parotidectomy(SP)on patients with superficial benign tumors of the parotid gland.Method A retrospective analysis was conducted on 68 patients with benign tumors in the superficial lobe of the parotid gland admitted to Luoyang Central Hospital affiliated with Zhengzhou University from May 2016 to May 2021.They were divided into two groups according to different treatment methods,including 34 cases of PSP surgery and 34 cases of SP surgery.The surgical indicators,preoperative and postoperative related scores[body image concern inventory(BICI),body image scale(BIS),hospital anxiety and depression scale(HADS)],postoperative complications and quality of life scale(SF-36)score of the two groups were observed.Results There was no statistically significant difference in BICI,BIS,HADS,and SF-36 scores between the two groups before surgery(P>0.05);The blood loss and the number of dissected facial nerve branches in PSP group were less than those in SP group,and the operation time and incision length were shorter than those in SP group;Before discharge,the scores of both groups decreased,with the scores of BICI,BIS,HADS anxiety,and HADS depression in the PSP group being lower than those in the SP group;The incidence of postoperative complications in the PSP group(14.71%)was lower than that in the SP group(35.29%);Before discharge,the SF-36 score in the PSP group was higher than that in the SP group,with a statistically significant difference(P<0.05).Conclusion Partial parotidectomy for benign tumors in the superficial lobe of the parotid gland can effectively shorten the surgical time,reduce intraoperative bleeding,improve patients'negative emotions,reduce the occurrence of various complications,improve patients'postoperative quality of life,and achieve ideal overall efficacy.
