结膜瓣遮盖术在治疗免疫性与感染性难治性角膜疾病中的效果比较

摘　　要：目的探讨结膜瓣遮盖术在难治性角膜疾病治疗中对治愈率的影响。方法回顾性分析安阳市眼科医院2014年6月至2019年6月收治的50例免疫性与感染性难治性角膜疾病病例。根据疾病种类分组,感染性角膜组39例,免疫性角膜组11例。所有病例均经同一医生进行结膜瓣遮盖术治疗,术后对原发疾病持续采取药物对症治疗。随访6个月,对比两组治愈率、并发症发生情况、视力恢复情况、生活质量量表(SF-36)评分。结果免疫性角膜组的治愈率高于感染性角膜组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。两组视力情况比较,免疫性角膜组视力>0.06占比率明显高于感染性角膜组,差异有统计学意义(P<0.05)。术前两组生理、心理职能及社会功能、精神健康评分相比,差异无统计学意义(P>0.05)。术后6个月两组生活质量评分相比,免疫性角膜组评分较高,差异有统计学意义(P<0.05)。结论结膜瓣遮盖术治疗各种类型难治性角膜疾病的疗效显著,并且免疫性角膜疾病的治愈率高于感染性角膜疾病,视力和生活质量优于感染性角膜病患者。Objective To investigate the effect of conjunctival flap covering on the cure rate in the treatment of refractory corneal diseases.Methods A retrospective analysis was performed on 50 patients with refractory immune and infectious corneal diseases admitted to Anyang Eye Hospital from June 2014 to June 2019.According to the types of diseases,39 cases were infected cornea group and 11 cases were immune cornea group.All cases were treated with conjunctival flap occlusion by the same doctor,and the primary disease was treated with symptomatic medication continuously after surgery.After 6 months of follow-up,the cure rate,complications,visual recovery,and quality of life scale(SF-36)score were compared between the two groups.Results The cure rate of the immune cornea group was higher than the infectious cornea group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The visual acuity of the two groups was compared,the visual acuity>0.06 in the immune cornea group was significantly higher than that in the infectious cornea group,and the difference was statistically significant(P<0.05).There were no significant differences in physiological and psychological functions,social functions,and mental health scores between the two groups before surgery(P>0.05).At 6 months after surgery,the quality of life score of the immune corneal group was higher than that of the infectious cornea group,and the difference was statistically significant(P<0.05).Conclusion The conjunctival flap is effective in the treatment of various types of refractory corneal diseases,and the cure rate of immune corneal diseases is higher than that of infectious corneal diseases,and the vision and quality of life are better than those of infectious corneal diseases.
