基于“阳化气,阴成形”探析泛血管疾病的辨治思路

摘　　要：基于《黄帝内经》“阳化气,阴成形”理论,进一步阐发其内涵为阴阳和合、动静结合、体用联合。结合泛血管疾病的特点及临床表现,从“阳化气”与“阴成形”的太过与不及分析病机,认为“阳化气”与“阴成形”不及所致的浊邪侵袭而化源修复不利是泛血管疾病的基本病机,而“阳化气”与“阴成形”的太过是发生变证和疾病进展的重要条件。结合临床经验,初步提出“温、通、补、动”四法辨治泛血管疾病,其中“温”调体质使命门启发、三焦气化,“通”为治标使三焦分治而有的放矢,“补”为治本使阴阳互求而形神相守,“动”为宗旨其关键在于三焦恒动、风药助动。以“阳化气,阴成形”指导泛血管疾病的中医药治疗,有助于深入认识该病病因病机并拓宽临床诊疗思路。The connotation of“yang transforming into qi,while yin constituting form”theory is further discussed based on Inner Canon of Yellow Emperor(《黄帝内经》),i.e.balance and harmony of yin and yang,combination of dynamic and static,and combination of form and function.Combined with the characteristics and clinical manifestations of panvascular disease,the pathogenesis of the disease can be analyzed from the excess and deficiency of“yang transforming into qi”and“yin constituting form”.Turbid pathogen caused by the deficiency of“yang transforming into qi”and“yin constituting form”and disfunction of body self-repair is the basic pathogenesis of panvascular disease.And the excess of them is an important condition for pattern change and the progress of the disease.Combined with clinical experience,we put forward the four methods of“warming,unblocking,supplementing,and moving”to differentiate and treat panvascular disease.Among them,the“warming”helps regulate constitution to activate ming men(gate of vitality,命门)and qi transformation of sanjiao(three energizers,三焦),“unblocking”is targeted for treating symptoms and being aim-oriented treatment,“supplementing”treats root causes to achieve mutual improvement of yin and yang and concomitance of form and spirit,and“moving”is the main idea the movement of sanjiao and the effect of wind-dispelling medicinals.With the guide of“yang transforming into qi”and“yin constituting form”to treat panvascular disease can help deeply understand the etiology and pathogenesis of the disease and broaden clinical diagnosis and treatment ideas.
