摘  要:目的应用Corvis ST评估不同近视程度成年患者的角膜生物力学及及与年龄、近视程度的相关性。方法横断面研究。收集2021年1月至8月在佛山市第二人民医院眼科门诊就诊的拟行屈光手术的近视患者101例(101眼,右眼)。根据等效球镜屈光度(SE)分为轻度近视组(SE>-3.00 D)组(12眼)、中度近视组(-6.00 D0.05)。轻度近视组患者的HC-DA均低于中度近视组、高度近视组和超高度近视组,差异均有统计学意义(P_(轻-中)=0.002,P_(轻-高)=0.004,P_(轻-超高)=0.016),其他组间差异均无统计学意义(均为P>0.05)。轻度近视组患者的SP-A1均高于中度近视组、高度近视组和超高度近视组,差异均有统计学意义(P_(轻-中)=0.016,P_(轻-高)=0.002,P_(轻-超高)=0.003),其他组间差异均无统计学意义(均为P>0.05)。轻度近视组患者IR低于中度近视组、超高度近视组,差异均有统计学意义(P_(轻-中)=0.021,P_(轻-超高)=0.013),其他组间差异均无统计学意义(均为P>0.05)。轻度近视组患者DA Ratio低于中度近视组,差异有统计学意义(P=0.038),其他组间差异均无统计学意义(均为P>0.05)。以CCT和bIOP为控制变量,角膜上下方平均屈光力的差值与年龄呈正相关(P=0.018),HC-PD、HC-DA与SE均呈负相关(均为P<0.05)。无控制变量时,第1次压平长度、HC-DA与年龄均呈正相关(均为P<0.05);SP-A1与SE呈正相关(P=0.030)。结论DA Ratio、IR可能较好地反映轻度近视患者的角膜生物力学。HCR在超高度近视患者中明显变小,提示超高度近视患者的角膜生物力学变弱,易发生形变。Objective To evaluate the corneal biomechanical parameters of patients with varying degrees of myopia and their relation with the age of patients and severity of myopia using corneal visualization scheimpflug technology(Corvis ST).Methods In this cross-sectional study,101 right eyes of 101 patients who visited the Ophthalmology Clinic,Foshan Second People’s Hospital and planned to undergo refractive surgery from January to August 2021 were included.Based on the spherical equivalent(SE),patients were divided into the mild myopia group(SE>-3.00 D)(12 eyes),medium myopia group(-6.00 D<SE≤-3.00 D)(39 eyes),high myopia group(-10.00 D<SE≤-6.00 D)(39 eyes),and ultrahigh myopia group(SE≤-10.00 D)(11 eyes).The corneal biomechanical parameters of patients in all groups were measured and compared by IOLMaster 700,Pentacam and Corvis ST.With central corneal thickness(CCT)and biomechanically corrected intraocular pressure(bIOP)as controlled variables,the relationship between age,SE and corneal biomechanical parameters was analyzed using partial correlation analysis.The stepwise regression analysis was performed separately to explore the impact of age on various biomechanical parameters after the single-factor screening.Results There were significant differences in the overall comparison and pairwise comparison of axial length(AL)and peak distance at the highest concavity(HC-PD)(all P<0.05).There were significant differences in the overall comparison of SE,highest concavity radius(HCR),deformation amplitude at the highest concavity(HC-DA),stiffness parameter applanation 1(SP-A1),integrated radius(IR),and deformation amplitude ratio(DA Ratio)at 2 mm(all P<0.05).There were significant differences in the SE between the mild myopia group and high myopia group,mild myopia group and ultrahigh myopia group,medium myopia group and high myopia group,as well as medium myopia group and ultrahigh myopia group(all P<0.05),and no significant difference in the SE between the mild myopia group and medium myopia group,high myopia group
