摘　　要：目的:研究多发性骨髓瘤经外周静脉穿刺置入中心静脉导管(PICC)置管化疗患者病耻感现况及相关影响因素,引起医护人员对于多发性骨髓瘤PICC置管化疗患者病耻感的关注,为临床改善病耻感提供可参考依据。方法:以我院2019年6月—2022年5月收治的203例多发性骨髓瘤PICC置管化疗患者为研究对象,采用单因素分析影响多发性骨髓瘤PICC置管化疗患者社会影响量表(SIS)评分的因素,并对有差异的因素赋值进行多元线性分析。结果:病耻感总分(69.13±5.27)分,处于中等水平;因病中断工作、病程、化疗周期、临床分期、重度贫血、转移、复发、达到CR、SSRS评分、SUPPH评分、体能状态不同的多发性骨髓瘤PICC置管化疗患者病耻感评分比较,差异有统计学意义(P<0.05);多元线性分析显示,因病中断工作、病程、化疗周期、临床分期、重度贫血、转移、复发、达到CR、SSRS评分、SUPPH评分、体能状态为多发性骨髓瘤PICC置管化疗患者病耻感评分的影响因素(P<0.05)。结论:多发性骨瘤PICC置管化疗患者病耻感处于中等水平,受多个因素影响,临床可针对影响因素制定干预措施以改善病耻感。Objective:To study the current situation of stigma in patients with multiple myeloma treated with chemotherapy via peripheral venous puncture with central venous catheter(PICC)placement and the related influencing factors,to draw the attention of healthcare professionals to the stigma of patients with multiple myeloma treated with PICC placement,and to provide a reference basis for clinical improvement of stigma.Methods:A total of 203 multiple myeloma patients with PICC catheterization for chemotherapy admitted to our hospital from June 2019 to May 2022 were selected as the research objects.Single factor analysis was used to analyze the factors affecting the social impact scale(SIS)score of multiple myeloma patients with PICC catheterization for chemotherapy,and multivariate linear analysis was performed on the assignment of different factors.Results:The total score of stigma was(69.13±5.27)scores,which was at a medium level.There were statistically significant differences in the scores of stigma in patients with multiple myeloma PICC catheterization chemotherapy with different disease interruption,course of disease,chemotherapy cycle,clinical stage,severe anemia,metastasis,recurrence,CR,SSRS score,SUPPH score and physical status(P<0.05).Multivariate linear analysis showed that interruption of work due to illness,course of disease,chemotherapy cycle,clinical stage,severe anemia,metastasis,recurrence,CR,SSRS score,SUPPH score and physical status were the influencing factors of stigma score in patients with PICC catheterization chemotherapy for multiple myeloma(P<0.05).Conclusion:The stigma of multiple myeloma patients with PICC catheter for chemotherapy is at a moderate level,which is affected by many factors.Clinical intervention measures can be formulated according to the influencing factors to improve the stigma.
