达格列净联合维格列汀治疗2型糖尿病老年患者的效果

摘　　要：目的:研究达格列净联合维格列汀治疗老年2型糖尿病(T2DM)效果及对血清蛋白酪氨酸磷酸酶1B(PTP1B)和胰高血糖素素样肽-1(GLP-1)的影响。方法:以2021年1月—2022年4月我院收治的206例老年2型糖尿病(T2DM)患者为研究对象,根据随机数字表法将患者分为对照组、研究组,每组103例。对照组采用达格列净治疗,研究组采用达格列净联合维格列汀治疗。比较两组临床疗效、糖脂代谢水平[空腹血糖(FPG)、餐后2h血糖(2hPG)、总胆固醇(TC)、三酰甘油(TG)]、胰岛素β细胞功能指标[胰岛β细胞功能指数(HOMA-β)、空腹C肽(FCP)、餐后C肽(PCP)]、血清炎症因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)]及氧化应激因子[总抗氧化能力(T-AOC)、脂质过氧化氢(LHP)]水平、血清PTP1B、GLP-1水平。结果:研究组总有效率为97.09%,高于对照组的88.35%(P<0.05);研究组FPG、2hPG、TC、TG、hs-CRP、IL-6、LHP、PTP1B水平低于对照组,HOMA-β、FCP、PCP、T-AOC、GLP-1水平高于对照组(P<0.05)。结论:达格列净联合维格列汀治疗老年T2DM的效果显著,可有效改善胰岛素β细胞功能,调节糖脂代谢,缓解临床症状,促进病情恢复。Objective:To study the effect of dapagliflozin combined with vildagliptin in the treatment of elderly patients with type 2 diabetes mellitus(T2DM)and its effect on serum protein tyrosine phosphatase 1B(PTP1B)and glucagon-like peptide-1(GLP-1).Methods:A total of 206 elderly patients with type 2 diabetes mellitus(T2DM)admitted to our hospital from January 2021 to April 2022 were selected as the research objects.According to the random number table method,the patients were divided into control group and study group,with 103 cases in each group.The control group was treated with dapagliflozin,and the study group was treated with dapagliflozin combined with vildagliptin.The clinical efficacy,glucose and lipid metabolism levels[fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),total cholesterol(TC),triglyceride(TG)],insulinβ-cell function indexes[isletβ-cell function index(HOMA-β),fasting C-peptide(FCP),postprandial C-peptide(PCP)],serum inflammatory factors[hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6)]and oxidative stress factors[total antioxidant capacity(T-AOC),lipid hydrogen peroxide(LHP)]levels,serum PTP1 B,GLP-1 levels were compared between the two groups.Results:The total effective rate of the study group was 97.09%,which was higher than 88.35%of the control group(P<0.05).The levels of FPG,2 hPG,TC,TG,hs-CRP,IL-6,LHP and PTP1 B in the study group were lower than those in the control group,and the levels of HOMA-β,FCP,PCP,T-AOC and GLP-1 were higher than those in the control group(P<0.05).Conclusion:Dapagliflozin combined with vildagliptin has a significant effect in the treatment of elderly T2DM,which can effectively improve insulinβcell function,regulate glucose and lipid metabolism,relieve clinical symptoms and promote disease recovery。
