益肾活血方联合中药熏蒸治疗硼替佐米致周围神经病变的临床效果

摘　　要：目的探讨益肾活血方联合中药熏蒸治疗硼替佐米致周围神经病变(BIPN)的临床效果。方法选取74例BIPN患者作为研究对象,采用随机数字表法将患者分为观察组和对照组,每组37例。对照组采用甲钴胺治疗,观察组在对照组治疗基础上加用益肾活血方联合中药熏蒸治疗,两组均治疗4周。比较两组患者治疗后的临床疗效,以及治疗前后神经毒性评分、Karnofsky功能状态(KPS)评分、疼痛数字评价量表(NRS)评分、神经传导速度。结果观察组总有效率高于对照组(P<0.05)。治疗后,两组的神经毒性评分、疼痛NRS评分均低于治疗前,且观察组的评分低于对照组(均P<0.05);治疗后两组的KPS评分均高于治疗前,且观察组的评分高于对照组(均P<0.05);观察组正中神经运动神经传导速度(MNCV)、两组正中神经感觉神经传导速度(SNCV)、两组腓总神经MNCV和SNCV的神经传导速度均较治疗前增快,且观察组快于对照组(均P<0.05)。结论在甲钴胺治疗基础上,加用益肾活血方联合中药熏蒸可提高BIPN的治疗效果,减轻神经毒性及疼痛,改善患者的生活质量,提高神经传导速度,效果优于单独使用甲钴胺治疗,值得在临床中推广。Objective To investigate the clinical effect of Yishen Huoxue Prescription combined with Traditional Chinese Medicine fumigation in the treatment of bortezomib-induced peripheral neuropathy(BIPN).Methods A total of 74 patients with BIPN were selected as the research subjects,and they were assigned to observation group or control group by employing the random number table method,with 37 cases in each group.The control group was treated with mecobalamin,based on which the observation group was treated additionally with Yishen Huoxue Prescription combined with Traditional Chinese Medicine fumigation.Both groups received a 4-week treatment.The post-treatment clinical efficacy,and pre-and post-treatment neurotoxicity score,Karnofsky Performance State(KPS)score,Numerical Rating Scale(NRS)score for pain,and nerve conduction velocity in patients were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the neurotoxicity score,NRS score for pain of the two groups were all lower than those before treatment,and the observation group yielded lower scores as compared with the control group(all P<0.05).After treatment,the KPS score of both groups was higher than that before treatment,and the observation group exhibited a higher score as compared with the control group(all P<0.05);in addition,the motor nerve conduction velocity(MNCV)of median nerve in the observation group,sensory nerve conduction velocity(SNCV)of median nerve in both groups,and MNCV and SNCV of common peroneal nerve in both groups were all faster than those before treatment,and the observation group interpreted faster nerve conduction velocity as compared with the control group(all P<0.05).Conclusion On the basis of mecobalamin therapy,additionally employing Yishen Huoxue Prescription combined with Traditional Chinese Medicine fumigation can improve the therapeutic effect of BIPN,relieve neurotoxicity and pain,ameliorate patients′quality of life,improve ner
