摘　　要：目的 评价清热除湿中药灌肠方对溃疡性结肠炎(ulcerative colitis,UC)患者快速诱导缓解和改善症状的有效性。方法 随机选取2019年2月—2021年2月山东省寿光市人民医院收治的60例UC患者为研究对象。以随机抽签法分为对照组(30例,以美沙拉嗪治疗)和观察组(30例,结合清热除湿中药灌肠方治疗)。对比两组症状缓解,炎症因子指标水平及安全性。结果 观察组整体快速诱导缓解率(96.6%)高于对照组(73.3%),差异有统计学意义(χ^(2)=4.706,P=0.030);两组治疗后的炎性因子水平均有明显下降,且观察组CRP、TNF-α、IL-6及IL-8指标均低于对照组,差异有统计学意义(P<0.05);观察组用药不良反应发生率(6.7%)低于对照组(33.3%),差异有统计学意义(χ^(2)=6.667,P=0.010)。结论 清热除湿中药灌肠方联合美沙拉嗪治疗溃疡性结肠炎,在患者症状缓解、内镜梅奥评分、长期控制腹痛、便血等症状方面均优于单纯应用美沙拉嗪;且联合治疗方案可加强发挥清除炎性因子作用,降低炎性细胞对患者结肠黏膜的刺激,促进结肠功能尽早恢复。Objective To evaluate the effectiveness of the Chinese herbal enema formula for clearing heat and removing dampness to rapidly induce relief and improve symptoms in patients with ulcerative colitis(UC).Methods 60 cases of UC patients admitted to Shouguang People's Hospital from February 2019 to February 2021 were randomly selected for the study.They were divided into a control group(30 cases,treated with mesalazine)and an observation by random sampling group(30 cases,treated with a combination of clearing heat and removing dampness Chinese herbal enema formula).The symptom relief,levels of inflammatory factors and safety of the two groups were compared.Results The overall rapid induction remission rate in the observation group(96.6%)was higher than that in the control group(73.3%),and the difference was statistically significant(χ^(2)=4.706,P=0.030).The decreased significantly in two groups after treatment,and the indexes of CRP,TNF-α,IL-6 and IL-8 in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse drug reactions in the observation group(6.7%)was lower than that in the control group(33.3%),and the difference was statistically significant(χ^(2)=6.667,P=0.010).Conclusion The treatment of ulcerative colitis with the combination of clearing heat and removing dampness herbal enema formula and mesalazine was superior to the application of mesalazine alone in terms of symptom relief,endoscopic Mayo score,and long-term control of abdominal pain and blood in stool in patients.Moreover,the combined treatment regimen could enhance the effect of scavenging inflammatory factors,reduce the irritation of inflammatory cells on patients'colonic mucosa,and promote the early recovery of colonic function.
