绑缚术联合中药治疗直肠前突型便秘疗效观察

摘　　要：目的 评价绑缚术联合益气养阴中药治疗气阴两虚型直肠前突型便秘的临床疗效。方法 将2020年11月—2021年12月于上海中医药大学附属龙华医院肛肠科门诊就诊的74例气阴两虚型直肠前突型便秘患者随机分为2组,治疗组37例采用绑缚术联合益气养阴中药治疗,对照组37例采用生物反馈疗法联合益气养阴中药治疗,2组均以28 d为1个疗程,共治疗1个疗程。记录治疗前及治疗7,14,28,90 d后2组患者排便时间、每周手助排便次数、出口梗阻综合证(ODS)评分量表评分及中医证候积分,评估2组临床疗效。结果 治疗7,14,28,90 d后,2组排便时间均明显短于治疗前,每周手助排便次数少于治疗前,ODS评分、中医证候积分均明显低于治疗前,差异均有统计学意义(P均<0.05);治疗组治疗后各时间点排便时间均明显短于对照组,每周手助排便次数均明显少于对照组,ODS评分、中医证候积分均明显低于对照组,差异均有统计学意义(P均<0.05)。2组患者中医证候总有效率均为97.30%(36/37),治疗组痊愈率为13.51%(5/37),明显高于对照组,治疗组疗效优于对照组(P<0.05)。结论 绑缚术联合益气养阴中药可缩短气阴两虚型直肠前突型便秘患者的排便时间,减少手助排便次数,改善出口梗阻综合征及中医证候。Objective It is to evaluate the clinical efficacy of binding therpay combined with Chinese herbs for supplementing qi and nourishing yin in the treatment of rectocele constipation with syndrome of deficiency of both qi and yin.Methods Seventy-four patients with rectocele constipation with syndrome of deficiency of both qi and yin who visited the Anorectal Clinic of Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from November 2020 to December 2021 were randomly divided into two groups.The treatment group(n=37)was treated with binding surgery combined with Chinese herbs for supplementing qi and nourishing yin,and the control group(n=37)was treated with biofeedback therapy combined with Chinese herbs for supplementing qi and nourishing yin.28 days as 1 course of treatment,and both groups were treated for 1 course of treatment.The duration of defecation,the number of hand-assisted defecation per week,the scores of obstructive defecation syndrome(ODS)scale and the score of traditional Chinese medical symptoms were recorded before and after treatment for 7,14,28,90 days.The clinical efficacy of the 2 groups was evaluated.Results After 7,14,28,and 90 days of treatment,the defecation times of both groups were significantly shorter than those before treatment,and the numbers of hand-assisted defecation per week were less than those before treatment,the ODS scores and traditional Chinese medical syndrome scores were significantly lower than those before treatment,and the differences were all statistically significant(all P<0.05).The defecation time was significantly shorter,the numbers of hand-assisted defecation per week was more,the ODS scores were and traditional Chinese medical syndrome scores were lower than those in the control group at each time point,and the differences were all statistically significant(all P<0.05).The total effective rates of traditional Chinese medicine syndrome in both groups were both 97.3%(36/37),and the cure rate in the treatment group was 13.51%(5/37),whi
