摘 要:目的:探讨低强度激光(LILT)对正畸牙齿移动的疗效及龈沟液白细胞介素-1β(IL-1β),核因子κB受体激活剂配体(RANKL)和骨保护素(OPG)水平的影响。方法:纳入正畸治疗患者60例,随机分为试验组和对照组各30例,对照组使用牙齿正畸治疗,实验组加用LILT治疗。比较治疗后牙齿累积移动距离(CMD)和平均回缩速度(ARS),以及不同时间段疼痛VAS评分和龈沟液IL-1β,RANKL和OPG水平。结果:试验组7、14、21 d,28 d牙齿CMD均大于对照组(P<0.05),7、14和21 d ARS均高于对照组(P<0.05),治疗后4、24 h,7、14、21 d VAS评分均低于对照组(P<0.05),7 d OPG低于对照组,7、14和21 d RANKL高于对照组,21 d和28 d IL-1β高于对照组,7 d RANKL/OPG比率高于对照组(均P<0.05)。结论:LILT可通过影响骨代谢加速正畸牙齿的移动,并缓解治疗过程中的牙齿疼痛与牙周炎症。Objective:To investigate the effects of low intensity laser(LILT)on orthodontic tooth movement and the expression level of interleukin-1β(IL-1β),nuclear factorκB receptor activator ligand(RANKL)and osteoprotegerin(OPG)in gingival crevicular fluid(GCF).Methods:60 orthodontic patients were randomly divided into a test group and a control group(n=30).The patients in control group received orthodontic treatment,and those in the test group received orthodontic treatment with additional LILT treatment.The cumulative moving distance(CMD)and average retraction speed(ARS)of teeth after treatment,the pain VAS and gingival crevicular fluid IL-1β,RANKL and OPG level in different time periods were measured and compared.Results:Compared with the indexes of the control group,the CMD of teeth in the test group at 7,14,21 and 28 days after treatment was higher(P<0.05),the ARS of teeth at 7,14 and 21 days was higher(P<0.05),the pain VAS was lower at 4 and 24 h,7,14 and 21 days(P<0.05),OPG was lower(P<0.05)at 7 d,RANKL at 7 d,14 and 21 d was higher(P<0.05),IL-1βwas higher at 21 d and 28 d(P<0.05),and the ratio of RANKL/OPG at 7 d was higher(P<0.05).Conclusion:LILT can accelerate the movement of orthodontic teeth by affecting bone metabolism,and can relieve tooth pain and periodontal inflammation during treatment.
