角膜塑形镜在不同近视屈光度儿童中的应用效果研究

摘　　要：目的探讨角膜塑形镜在不同近视屈光度儿童中的应用效果。方法抽取2020年8月至2022年1月平煤神马医疗集团总医院收治的近视儿童45例(90眼),按照不同近视屈光度分为低度组(屈光度<-3.0 D)22例(44眼)和中高度组(等效球镜度为≥-3.0 D)23例(46眼)。两组均在常规眼科检查后佩戴角膜塑形镜治疗。比较两组治疗前、治疗6个月后、治疗18个月后裸眼视力、角膜曲率(平K值、陡K值)、泪膜破裂时间(BUT)、等效球镜屈光度及眼轴长度增长量变化。结果治疗6、18个月后,两组裸眼视力均较治疗前提升,且中高度组裸眼视力高于低度组,两组平K值、陡K值均较治疗前降低,且中高度组低于低度组(P<0.05);治疗6、18个月后,两组BUT均较治疗前降低(P<0.05),但两组间比较差异未见统计学意义(P>0.05);治疗6、18个月后,两组等效球镜屈光度增长量均较治疗前降低,且中高度组低于低度组(P<0.05);治疗6、18个月后,两组眼轴长度增长量均较治疗前降低,且中高度组低于低度组(P<0.05)。结论与近视屈光度较低的儿童相比,近视屈光度在-3.0 D以上的患儿使用角膜塑形镜的控制效果更好,能有效提升患儿裸眼视力,控制眼轴增长,但会在一定程度上降低泪膜稳定性。Objective To study the application of orthokeratology lens in children with different myopia diopter.Methods A total of 45 myopic children(90 eyes)treated in the General Hospital of Pingmei Shenma Medical Group from August 2020 to January 2022 were selected,and they were divided into low-degree group(diopter<-3.0 D,22 cases,44 eyes)and medium-high group(spherical equivalent≥-3.0 D,23 cases,46 eyes).Both groups were treated by orthokeratology lens after routine ophthalmic examination.The changes of uncorrected visual acuity,corneal curvature(flat meridian,steep meridian),tear break-up time(BUT),spherical equivalent diopter,and axial length of the two groups before treatment,after 6 months and 18 months of treatment were compared.Results After 6 months and 18 months of treatment,the uncorrected visual acuity of both groups were improved,compared with those before treatment,and the uncorrected visual acuity of the medium-high group was higher than that of the low-degree group,the flat meridian value and steep meridian value of the two groups were lower than those before treatment,and the two indexes of the medium-high group was lower than those of the low-degree group(P<0.05).After 6 months and 18 months of treatment,the BUT in both groups were lower than those before treatment(P<0.05),but there was no significant difference between the two groups(P>0.05).After 6 months and 18 months of treatment,the increase of spherical equivalent diopter in both groups were lower than those before treatment,and the increase of spherical equivalent diopter in the medium-high group was lower than that in the lower-degree group(P<0.05).After 6 months and 18 months of treatment,the increase of axial length in both groups were lower than those before treatment,and the index in the medium-high group was lower than that in the lower-degree group(P<0.05).Conclusions Compared with children with lower myopic diopter,orthokeratology lens had better control effects on children with myopic diopter above-3.0 D.It can effectively improve the
