摘　　要：目的探讨髓系肉瘤(MS)的临床表现、治疗及预后。方法对2011年2月至2022年2月11例MS患者的临床病理资料、诊断、治疗及预后进行回顾性分析。结果11例患者中男性6例,女性5例;年龄范围12~71岁;大部分患者以无痛性局部肿块或肿块引起的局部压迫症状为首发表现。11例患者中,MS同时合并急性髓系白血病(AML)2例;继发于AML者6例;孤立性MS 1例;MS伴慢性粒细胞白血病(CML)慢性期1例;真性红细胞增多症(PV)转化为AML伴MS 1例。8例行骨髓基因及染色体检查,其中5例存在基因突变或异常染色体核型。8例接受AML常规方案化疗、靶向药物或联合放疗,1例接受手术联合化疗,1例放弃治疗,1例因疾病迅速进展死亡而未治疗。11例患者中2例失访,7例死亡,2例存活。全组中位总生存时间为7.6个月(范围:0.8~62.4个月)。结论MS起病多样,临床表现不典型,治疗方案包括全身化疗、手术、放疗及靶向治疗等。MS患者的生存时间较短,疗效差。Objective To investigate the clinical manifestations,treatment and prognosis of myeloid sarcoma(MS).Methods The clinicopathological feature,diagnosis,treatment and outcome of 11 MS patients from February 2011 to February 2022 were retrospectively analyzed.Results Of the 11 MS patients,6 were male and 5 were female,ranged from 12 to 71 years old.In most patients,painless local mass or local compression caused by mass were the first symptoms.Among the 11 patients,2 cases coexisted with acute myeloid leukemia(AML);6 cases were secondary to AML;1 case was of solitary MS;1 case was chronic myelogenous leukemia(CML)in chronic phase with MS;1 case was secondary leukemia after polycythemia vera(PV)with MS.Of the 8 patients who underwent bone marrow gene and chromosome examination,5 patients showed gene mutation or abnormal chromosome karyotype.Eight patients received AML conventional chemotherapy,targeted drugs or combined radiotherapy,one patient received surgical treatment combined with chemotherapy,one patient gave up treatment,and one patient died of rapid disease progression without treatment.Two patients were lost to follow-up.Among the 9 patients who were followed-up,7 died and 2 survived;the median overall survival was 7.6 months(ranged 0.8-62.4 months).Conclusion MS has diverse onset and atypical clinical manifestations,treatment options including systemic chemotherapy,surgery,radiotherapy and target therapy.The survival of MS patients is short and the curative effect is poor.
