CT体积测量在结肠癌术前分期评估中的应用价值

摘　　要：目的探讨CT体积测量在结肠癌术前分期评估中的应用价值。方法选取经病理证实的108例结肠癌患者CT资料,测量静脉期肿瘤体积,评估结肠癌体积和相应TNM分期的相关性。绘制ROC曲线计算曲线下面积(AUC),并与CT主观评价分期相比较,分析结肠癌CT体积测量区分不同TNM分期的诊断效能。结果结肠癌CT体积与相应的T、N、M分期呈正相关。分析ROC曲线,CT体积测量区分≤T2和≥T3期、≤T3和T4期的AUC(0.956、0.892)、特异度(0.818、0.982)高于CT主观评价分期(AUC:0.698、0.733,特异度:0.455、0.655);CT体积测量区分N0和≥N1期的AUC(0.862)、灵敏度(0.857)高于CT主观评价分期(AUC:0.755、灵敏度:0.571)。CT体积测量区分M0和M1期的灵敏度(0.833)高于CT主观评价分期(0.750)。结论CT体积测量对结肠癌术前分期评估有重要应用价值,有利于指导临床选择个体化的治疗方案。Objective To analyze the application value of CT volumetry in preoperative staging of colon cancer.Methods CT data of 108 cases of colon cancer proven pathologically were collected and analyzed.The tumor volume was calculated in the venous phase,and the correlation between the results and TNM staging was evaluated.ROC curve was drawn and the area under the ROC curve(AUC)was calculeted to get the diagnostic value of CT volumetry for colon cancer to differentiate T,N and M staging.The diagnostic value of CT volumetry and subjective CT stage were compared.Results CT volume of colon cancer was positively correlated with TNM stage.By analyzing ROC curve,AUC(0.956,0.892)and specificity(0.818,0.982)of CT volumetry to differente between stage≤T2 and stage≥T3,stage≤T3 and stage T4 were higher than those of subjective CT stage(AUC:0.698,0.733.specificity:0.455,0.655).About differentiating between stage N0 and stage≥N1,the AUC(0.862)and the sensitivity(0.857)of CT volumetry were higher than those of subjective CT stage(AUC:0.755,sensitivity:0.571).About differentiating between stage M0 and stage M1,the sensitivity of CT volumetry(0.833)was higher than that of subjective CT stage(0.750).Conclusion CT volumetry for colon cancer plays an important part in predicting stageand optimizing individualized clinical strategy for patients.
