摘 要:目的 调查研究根管预备中镍钛器械分离的影响因素。方法 回顾性分析2014年6月—2018年6月海南医学院第一附属医院口腔科收治的120例牙髓病患者,均接受根管治疗。按照镍钛器械是否分离将患者分为两组,40例分离患者纳入观察组,80例未分离患者纳入对照组,分析影响镍钛器械分离的单因素和Logistic回归结果。结果 两组患者操作者经验、根管弯曲度、根管弯曲半径、器械锥度、机用器械转速、器械旋转方式、循环次数指标对比,差异有统计学意义(P<0.05)。Logistic多因素回归分析结果显示,影响显著的单因素分别为操作者经验、根管弯曲度、根管弯曲半径、器械锥度、机用器械转速、器械旋转方式、循环次数(OR=3.725、1.265、1.039、3.883、3.723、3.429、3.657)。结论 影响根管预备中镍钛器械分离的独立因素有操作者经验、器械锥度、根管弯曲度和弯曲半径等,在临床上操作时应当重视这些因素,采取对应措施帮助降低器械分离概率。Objective To investigate and study the factors influencing the separation of nickel-titanium instruments among root canal preparation.Methods Retrospective analysis of one hundred and twenty patients with endodontic disease who were admitted to the Department of Stomatology,the First Affiliated Hospital of Hainan Medical College from June 2014 to June 2018,all received root canal treatment.The patients were divided into two groups according to whether the nickel-titanium instruments were separated,40 separated patients were included in the observation group,and 80 patients without separation were included in the control group.The univariate and Logistic regression results affecting the separation of nickel-titanium instruments were analyzed.Results The indexes of operator experience,root canal curvature,root canal curvature radius,instrument taper,machine instrument speed,instrument rotation mode,and number of cycles between the two groups,the difference was statistically significant(P<0.05).Logistic multifactorial regression analysis showed that the significant single factors were:operator experience,root canal curvature,root canal curvature radius,instrument taper,rotational speed of the machine instrument,instrument rotation mode,and cycle number(OR=3.725,1.265,1.039,3.883,3.723,3.429,3.657).Conclusion Independent factors influencing the separation of nickel-titanium instruments in root canal preparation were operator experience,instrument taper,root canal curvature,and bending radius.These factors should be taken into account when operating in the clinic and corresponding measures should be taken to help reduce the probability of instrument separation.
