铋剂四联治疗胃溃疡的效果及对幽门螺杆菌根除率及细胞免疫指标的影响

摘　　要：目的 探讨胃溃疡(gastric ulcer, GU)应用铋剂四联治疗的效果及对幽门螺杆菌(Hp)根除率及细胞免疫指标的影响。方法 选择2019年10月至2021年1月商丘市第一人民医院收治的114例GU患者,根据电脑随机盲选法分为治疗组和对照组,每组57例。治疗前、治疗21天后,比较两组自然杀伤T细胞(NKT)、T淋巴细胞表面抗原-CD3^(+)、CD4^(+)、肠道菌群数量、胃动素(MTL)、胃泌素(GAS);比较治疗效果、Hp根除率、Hp复发率。结果 治疗21天后,两组NKT、CD3^(+)、CD4^(+)、双歧杆菌数、乳酸杆菌数、MTL均上升,且治疗组高于对照组,差异有统计学意义(P<0.05);两组大肠杆菌数、GAS均降低,且治疗组低于对照组,差异有统计学意义(P<0.05);治疗组治疗总有效率(92.98%)高于对照组(78.95%),差异有统计学意义(P<0.05);治疗组Hp根除率(94.49%)高于对照组(82.45%),差异有统计学意义(P<0.05);治疗组Hp复发率(3.51%)低于对照组(21.05%),差异有统计学意义(P<0.05)。结论 GU应用铋剂四联治疗,效果确切,可提高幽门螺杆菌根除率,改善细胞免疫指标水平,还可调节胃肠激素水平,改善肠道菌群数量,且疾病复发率低,远期预后好。Objective To investigate the effect of applying bismuth quadruple therapy for gastric ulcer(GU) and the effect on the eradication rate of Helicobacter pylori and cellular immune indexes. Methods One hundred and fourteen patients with GU admitted to Shangqiu the First People’s Hospital from October 2019 to January 2021 were selected and divided into treatment and control groups according to the computerized random blind selection method, with 57 cases in each group. Before treatment and after 21 days of treatment, the natural killer T cells(NKT), T lymphocyte surface antigen-CD3^(+), CD4^(+), intestinal flora count, gastrin(MTL) and gastrin(GAS) were compared between the two groups;the treatment effect, Hp eradication rate and Hp recurrence rate were compared. Results After 21 days of treatment, NKT, CD3^(+), CD4^(+), bifidobacterium count, lactobacillus count and MTL increased in both groups, and the treatment group was higher than that of the control group, the difference was statistically significant(P<0.05);E. coli count and GAS decreased in both groups, and the treatment group was lower than that of the control group, the difference was statistically significant(P<0.05);The total effective rate of treatment in the treatment group(92.98%) was higher than that of the control group(78.95%), and the difference was statistically significant(P<0.05);The eradication rate of Helicobacter pylori in the treatment group(94.49%) was higher than that of the control group(82.45%), and the difference was statistically significant(P<0.05);The recurrence rate in the treatment group(3.51%) was lower than that of the control group(21.05%), and the difference was statistically significant(P<0.05). Conclusion GU application of bismuth quadruple therapy is effective, can improve the eradication rate of Helicobacter pylori, improve the level of cellular immune index, also can regulate the level of gastrointestinal hormones, improve the number of intestinal flora, and the disease recurrence rate is low, the long-term prognosis is
