摘  要:目的:探讨γ-谷氨酰转移酶(γ-GT)与帕金森病(PD)患者病程、疾病严重程度的关系。方法:选取2019年3月至2020年11月在江苏盛泽医院神经内科住院的PD患者105例作为PD组,选取门诊患者、体检人员89例作为对照组。根据病程将PD组患者分为3个亚组,即<5年组(48例)、5~9年组(33例)和>9年(24例);根据Hoehn-Yahr评估结果将PD组患者分为早期组(40例,Ⅰ期、Ⅱ期患者)和中晚期组(65例,Ⅲ期、Ⅳ期、Ⅴ期患者)。收集各组的基线资料及血液检测相关指标。结果:PD组与对照组的TC、HDL、LDL和γ-GT水平相比,差异有统计学意义(P<0.05)。Logistic分析显示,γ-GT(OR=0.975,P=0.020)为PD保护因素。不同病程分组患者的性别、γ-GT水平相比,差异有统计学意义(P<0.05)。早期组与中晚期组的γ-GT水平相比,差异有统计学意义(P<0.05)。γ-GT(OR=0.930,P=0.032)为PD患者的中晚期保护因素。Spearman相关性分析显示,PD患者的γ-GT水平与病程呈负相关(r=-0.428、P=0.000),与Hoehn-Yahr分期无相关性(r=-0.164、P=0.137)。结论:γ-GT可作为PD早期筛查、诊断的一种生物标志物,γ-GT与PD患者的病程、疾病严重程度有关。Objective:To investigate the relationship between gamma-glutamylphthaltransferase(gamma-GT)and the course and severity of Parkinson’s disease(PD).Methods:A total of 105 PD patients hospitalized in the Department of Neurology of Jiangsu Shengze Hospital from March 2019 to November 2020 were selected as the PD group,and 89 outpatients and physical examiners were selected as the control group.According to the course of disease,PD group was divided into three subgroups,namely<5 years group(48 cases),5~9 years group(33 cases)and>9 years group(24 cases).According to the Hoehn-Yahr evaluation results,PD group was divided into early group(40 cases,stageⅠ,stageⅡ)and middle and late group(65 cases,stageⅢ,stageⅣ,stageⅤ).Baseline data and blood test indicators were collected.Results:There were significant differences in TC,HDL,LDL andγ-GT levels between PD group and control group(P<0.05).Logistic analysis showed that gamma-GT(OR=0.975,P=0.020)was a protective factor for PD.There were statistically significant differences in gender andγ-GT level among different disease course groups(P<0.05).There was statistical significance inγ-GT level between the early group and the middle and late group(P<0.05).Gamma-gt(OR=0.930,P=0.032)was a protective factor in patients with PD.Spearman correlation analysis showed that gamma-GT level in PD patients was negatively correlated with the course of disease(r=-0.428,P=0.000),and had no correlation with Hoehn-Yahr stage(r=-0.164,P=0.137).Conclusion:Gamma-GT can be used as a biomarker for early screening and diagnosis of PD.Gamma-gt is related to the course and severity of PD patients.
