摘 要:目的探讨高分辨率C臂CT联合减少金属伪影软件在Pipeline Flex治疗颅内动脉瘤的检查价值。方法分析诊断为颅内动脉瘤并使用Pipeline Flex植入治疗的18例患者临床资料。术后对Pipeline Flex植入区域分别行常规Xper-CT扫描及高分辨率头颅支架Xper-CT扫描,后者扫描得到的图像作常规图像重建和联合减少金属伪影技术重建。由两名神经介入医师分别对支架结构、支架贴壁情况以及支架与弹簧圈金属伪影程度进行主观量化分析。结果18名患者成功置入18枚支架,其中5例联合弹簧圈栓塞。与常规Xper-CT技术相比,高分辨率头颅支架Xper-CT常规重建及联合减少金属伪影技术重建在显示Pipeline Flex支架结构及打开方面差异无统计学意义(P>0.05),但其用于显示Pipeline Flex贴壁情况中,高分辨率头颅支架Xper-CT联合减少金属伪影技术重建使图像更加清晰;同时与颅内支架专用高分辨率Xper-CT常规重建相比,联合减少金属伪影技术重建在显示支架显影丝及减少弹簧圈伪影方面效果明显,图像质量明显改善(P<0.05)。结论高分辨率头颅支架Xper-CT扫描联合减少金属伪影技术在Pipeline Flex植入治疗术中具有很大的优势,这种独特的图像处理技术能有效降低支架及弹簧圈产生的金属伪影,提高支架贴壁情况的可视化,从而减少术中及术后不良事件的发生率。Objective To explore the value of high-resolution C-arm CT combined with metal artifact reduction software in the treatment of intracranial aneurysms by Pipeline Flex.Methods The clinical data of 18 patients diagnosed as intracranial aneurysms and treated with Pipeline Flex implantation were analyzed.Routine Xper-CT scan and high-resolution Xper-CT scan were performed on the implanted area of PipelineFlex after operation,respectively.The images obtained from the latter scan were reconstructed by routine image reconstruction and combined metal artifact reduction technique.Two neurologists performed subjective quantitative analysis on the stent structure,stent attachment and the degree of metal artifacts between stents and coils.Results 18 stents were successfully implanted in 18 patients,among which 5 cases were embolized with coil.Compared with conventional Xper-CT,there is no significant difference between conventional reconstruction of high-resolution Xper-CT and reconstruction of combined metal artifact reduction technology in showing the structure and opening of Pipeline Flex stent(P>0.05),but when it is used to show the adherence of Pipeline Flex,the reconstruction of high-resolution Xper-CT combined with metal artifact reduction technology makes the image clearer.At the same time,compared with the conventional reconstruction with high-resolution Xper-CT for intracranial stents,the combined reconstruction with metal artifact reduction technology has obvious effect in displaying stent developing filaments and reducing the artifacts of coils,and the image quality is obviously improved(P<0.05).Conclusion High-resolution Xper-CT scanning combined with metal artifact reduction technology has great advantages in Pipeline Flex implantation.This unique image processing technology can effectively reduce the metal artifacts produced by the stents and coils,improve the visualization of the stent attachment,and reduce the incidence of intraoperative and postoperative adverse events.
