脑胶质瘤患者MEK及ERK m RNA水平与病理特征及预后的关系

摘　　要：目的 探讨脑胶质瘤(BG)患者丝裂原活化细胞外信号调节激酶(MEK)、细胞外信号调节激酶(ERK)m RNA水平与病理及预后的关系。方法 选取2018-03—2019-03长沙市第四医院和中南大学湘雅医院的92例BG患者经手术切除的脑胶质瘤组织标本为研究对象(研究组),所有患者随访3 a,根据预后情况分为死亡组(n=35)及生存组(n=57),另取同期行颅内减压术切除治疗的60例脑损伤患者的正常脑组织标本为对照组,采用反转录聚合酶链反应(RT-PCR)检测MEK、ERK mRNA水平,比较不同临床病理特征BG患者组织MEK、ERK mRNA水平,采用ROC曲线分析组织MEK、ERK mRNA水平对BG患者预后的预测价值。结果 研究组MEK、ERK mRNA水平高于对照组(P<0.05)。MEK、ERK mRNA水平与BG患者WHO分级、肿瘤直径、瘤周脑水肿有关(P<0.05),WHO分级Ⅲ~Ⅳ级、肿瘤直径≥3 cm、瘤周脑水肿严重的BG患者组织MEK、ERK mRNA水平更高(P<0.05)。死亡组MEK、ERK mRNA水平高于生存组(P<0.05)。由ROC曲线可知,组织MEK、ERK mRNA水平联合预测BG患者预后的AUC为0.856,高于两者单独预测的0.779、0.763(P<0.05)。结论 BG患者组织MEK、ERK m RNA水平呈高表达,不同临床病理特征患者MEK、ERK mRNA水平存在差异,两者有望成为BG患者预后的预测指标。Objective To explore the relationship between mitogen-activated extracellular signal-regulated kinase(MEK),extracellular signal-regulated kinase(ERK)mRNA and pathology,prognosis in patients with brain glioma(BG).Methods The specimens of BG tissues from 92 patients with BG in the Changsha Fourth Hospital and Xiangya Hospital of Central South University between March 2018 and March 2019 were enrolled as study group.All were followed up for 3 years.According to prognosis,they were divided into death group(n=35)and survival group(n=57).The specimens of normal brain tissues from 60 patients with brain damage who underwent intracranial decompression during the same period were enrolled as control group.The m RNA levels of MEK and ERK were detected by reverse transcriptase polymerase chain reaction(RT-PCR),and which in BG patients with different clinicopathological characteristics were compared.The predictive value of MEK and ERK mRNA for prognosis was analyzed by ROC curves.Results The mRNA levels of MEK and ERK in study group were higher than those in control group(P<0.05).The mRNA levels of MEK and ERK were correlated with WHO grading,tumor diameter and peritumoral brain edema in BG patients(P<0.05).The mRNA levels of MEK and ERK were higher in BG patients with WHO grading at grade Ⅲ-IV,tumor diameter ≥3 cm and severe peritumoral brain edema(P<0.05).The mRNA levels of MEK and ERK in death group were higher than those in survival group(P<0.05).ROC curves analysis showed that AUC of MEK combined with ERK mRNA for predicting prognosis of BG patients was 0.856,greater than that of single index(0.779,0.763)(P<0.05).Conclusion The m RNA levels of MEK and ERK are high in BG patients.There are differences in levels of MEK and ERK mRNA among patients with different clinicopathological characteristics.The two indexes are expected to be prognostic indexes of BG patients.
