摘　　要：目的:探讨清胃化浊饮联合米曲菌胰酶片治疗功能性消化不良(FD)的临床效果。方法:选取2018年6月-2019年9月于河北省中医院脾胃病三科门诊就诊的130例FD患者,将其随机分为两组,治疗组65例予清胃化浊饮及米曲菌胰酶片,对照组65例予米曲菌胰酶片,两组用药均为60 d。比较两组治疗前后功能性消化不良生存质量量表(FDDQL)、7-分整体症状量表(GOSS)评分、血清胃泌素(GAS)、血清神经肽Y(NPY)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、胃窦收缩频率、胃窦收缩幅度、胃半排空时间及胃窦运动指数。结果:治疗后,治疗组GOSS评分、血清GAS、IL-6、TNF-α水平均低于对照组,FDDQL评分、血清NPY水平均高于对照组(P<0.05)。治疗后,治疗组胃窦收缩频率、胃窦收缩幅度、胃窦运动指数均高于对照组,胃半排空时间短于对照组(P<0.05)。结论:应用清胃化浊饮联合米曲菌胰酶片治疗FD,能够明显改善患者症状,促进胃排空,使患者生活质量得到改善,其机制可能与提高患者血清NPY水平,降低血清GAS、IL-6、TNF-α水平有关。Objective:To evaluate the clinical effect of Qingwei Huazhuo Decoction combined with Oryz-Aspergillus Enzyme and Pancreatin Tablets in the treatment of functional dyspepsia(FD).Method:A total of 130 FD patients admitted to the Third Department of Spleen and Stomach Diseases,Hebei Provincial Hospital of Traditional Chinese Medicine from June 2018 to September 2019 were selected,they were randomly divided into two groups,65 patients in the treatment group were given Qingwei Huazhuo Decoction combined with Oryz-Aspergillus Enzyme and Pancreatin Tablets,and 65 patients in the control group were given Oryz-Aspergillus Enzyme and Pancreatin Tablets,both groups were treated for 60 days.Functional digestive disorder quality of life questionnaire(FDDQL),7-point global overall symptom scale(GOSS)scores,serum gastrin(GAS),serum neuropeptide Y(NPY),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)levels,the frequency of gastric antrum contraction,amplitude of gastric antrum contraction,gastric semi-emptying time and gastric antrum motility index were compared between the two groups before and after treatment.Result:After treatment,the levels of GOSS score,serum GAS,IL-6 and TNF-αin treatment group were lower than those in control group,while the levels of FDDQL score and serum NPY in treatment group were higher than those in control group(P<0.05).After treatment,the frequency of gastric antrum contraction,amplitude of gastric antrum contraction and gastric antrum motility index in the treatment group were higher than those in the control group,and the gastric semi-emptying time was shorter than that in the control group(P<0.05).Conclusion:Qingwei Huazhuo Decoction combined with Oryz-Aspergillus Enzyme and Pancreatin Tablets in the treatment of FD can improve the symptoms,promote gastric emptying and improve the quality of life of patients,the mechanism may be related to the increase of serum NPY level and the decrease of serum GAS,IL-6 and TNF-αlevels.
