摘 要:目的研究口腔癌前病损转化微环境中白细胞介素-1β(IL-1β)/调节性T细胞(Treg)、辅助性T细胞7(Th17)/Treg、Th1/Th2免疫失衡的机制。方法选取60例口腔癌前病损患者纳入口腔癌前病损组,同时选取同时间段就诊的口腔癌患者43例和健康体检者35例,分别纳入口腔癌组及健康对照组。对比3组受试者IL-1β/Treg、Th17/Treg、Th1/Th2水平,并分析其与口腔癌前病变发生及进一步发展为口腔癌的关系及IL-1β/Treg、Th17/Treg、Th1/Th2三者的相关性。结果3组受试者IL-1β/Treg、Th17/Treg、Th1/Th2水平差异有统计学意义(P<0.05),其中IL-1β/Treg、Th17/Treg与口腔癌前病损的发生、恶化均呈明显正相关(P<0.05),而Th1/Th2则呈明显负相关(P<0.05)。口腔癌前病损组IL-1β/Treg、Th17/Treg水平呈正相关(P<0.05),与Th1/Th2呈负相关(P<0.05)。结论口腔微环境IL-1β/Treg免疫失衡可通过影响Th17/Treg、Th1/Th2表达水平促使正常组织转化为癌前病损并恶变为口腔癌,免疫微环境调控可作为临床防治口腔癌的靶点。Objective To study the mechanisms of immune imbalances of interleukin-1β(IL-1β)/regulatory T cell(Treg),T helper cell 7(Th17)/Treg and Th1/Th2 in the transformed microenvironment of oral precancerous lesions.Methods 60 patients with oral precancerous lesions were selected as oral precancerous lesion group.43 patients with oral cancer and 35 healthy people with physical examination during the same time period were included as oral cancer group and healthy control group respectively.The levels of IL-1β/Treg,Th17/Treg,and Th1/Th2 in the 3 groups of subjects were compared,and their relationship with occurrence of oral precancerous lesions and further development of oral cancer and correlation among levels of IL-1β/Treg Th17/Treg and Th1/Th2 were analyzed.Results There were statistically significant differences in the levels of IL-1β/Treg,Th17/Treg,and Th1/Th2 among the three groups(P<0.05).IL-1β/Treg and Th17/Treg were significantly positively correlated with the occurrence and aggravation of oral precancerous lesions(P<0.05),while Th1/Th2 was significantly negatively correlated(P<0.05).The IL-1β/Treg and Th17/Treg in oral precancerous lesions were positively correlated with each other(P<0.05),while the above 2indicators were negatively correlated with Th1/Th2(P<0.05).Conclusion The immune imbalance of IL-1β/Treg in the oral microenvironment can promote the transformation of normal tissues into precancerous lesions and malignancy into oral cancer by affecting the expression levels of Th17/Treg and Th1/Th2.The regulation of immune microenvironment can be used as a clinical target for the prevention and treatment of oral cancer.
