18F-FDGPET/CT对肝外胆管细胞癌术前诊断及分期的应用价值

摘　　要：目的探讨18氟-脱氧葡萄糖正电子发射电子计算机断层扫描术(18F-FDGPET/CT)对肝外胆管细胞癌(EHCC)术前诊断及分期的应用价值。方法回顾性分析96例疑为EHCC患者的临床资料。96例患者均采用18F-FDGPET/CT检查。统计病理学检查结果,计算18F-FDGPET/CT诊断EHCC结果,以及18F-FDGPET/CT诊断区域淋巴结转移及远处转移结果。结果96例疑为EHCC患者经病理学检查证实良性21例,恶性75例;18F-FDGPET/CT诊断EHCC敏感度为74.67%,特异度为80.95%,准确率为76.04%,诊断EHCC结果与病理结果一致性较为理想(Kappa=0.442);75例EHCC患者中出现区域淋巴结转移28例,远处转移4例;18F-FDGPET/CT诊断区域淋巴结转移敏感度为60.71%,特异度为89.36%;准确率为78.67%,诊断区域淋巴结转结果与病理结果一致性较为理想(Kappa=0.523);18F-FDGPET/CT诊断远处转移敏感度为75.00%,特异度为94.37%;准确率为93.33%,诊断区域淋巴结转结果与病理结果一致性较为理想(Kappa=0.512)。结论18F-FDGPET/CT作为无创性检查方法,对EHCC诊断效能较高,在明确区域淋巴结转、远处转移中更具优势,能够为EHCC术前诊断及分期提供确切信息。Objective To investigate the value of 18 fluorodeoxyglucose positron emission computed tomography(18f-fdgpet/CT)in extrahepatic cholangiocarcinoma(EHCC)application value of preoperative diagnosis and staging.Methods Clinical data of 96 patients suspected EHCC were analyzed retrospectively.All 96 patients were examined by 18f-fdgpet/CT.The pathological examination results were counted,the ehcc results diagnosed by 18f-fdgpet/CT.Results The regional lymph node metastasis and distant metastasis results diagnosed by 18f-fdgpet/CT were calculated Among 96 patients suspected EHCC,21 were benign and 75 were malignant;The sensitivity,specificity and accuracy of 18f-fdgpet/CT in the diagnosis of EHCC were 74.67%,80.95%and 76.04%,The consistency between the diagnosis of EHCC and the pathological results was ideal(kappa=0.442);28 cases of regional lymph node metastasis and 4 cases of distant metastasis occurred in 75 patients with EHCC;the sensitivity of 18f-fdgpet/CT in the diagnosis of regional lymph node metastasis was 60.71%,the specificity was 89.36%,and the accuracy was 78.67%.The consistency between the diagnosis of regional lymph node metastasis and the pathological results was ideal(kappa=0.523);the sensitivity of 18f-fdgpet/CT in the diagnosis of distant metastasis was 75.00%,the specificity was 94.37%,and the accuracy was 93.33%(kappa=0.512).Conclusion Conclusion 18f-fdgpet/CT,as a noninvasive method,has high diagnostic efficiency for EHCC,has more advantages in identifying regional lymph node metastasis and distant metastasis,and can provide accurate information for preoperative diagnosis and staging of EHCC.
