健脾清毒汤结合化疗对胃癌患者免疫球蛋白、毒副反应、COX-2、PGE2水平的影响

摘　　要：目的:探讨健脾清毒汤结合化疗对胃癌患者免疫球蛋白(Ig)、毒副反应、环氧合酶-2(COX-2)、前列腺素E2(PGE2)水平的影响。方法:选取2016年1月~2020年1月淮安市肿瘤医院收治的86例胃癌患者,根据随机数字表法分为对照组和观察组,各43例。对照组接受化疗治疗,观察组在对照组基础上加用健脾清毒汤治疗。对比两组患者临床疗效和治疗前后中医症状积分、健康状况、血清Ig和COX-2、PGE2水平及毒副反应发生情况。结果:观察组疾病控制率明显高于对照组(P<0.05);观察组中医症状积分和血清COX-2、PGE2水平及血小板下降、周围神经异常、丙氨酸氨基转移酶(ALT)升高、白细胞降低、血肌酐升高比例明显低于对照组,Karnofsky功能状态(KPS)评分和血清IgA、IgG、IgM水平明显高于对照组(P<0.05)。结论:健脾清毒汤结合化疗治疗胃癌具有增效减毒作用,可显著提升患者免疫力,改善健康状况,抑制COX-2、PGE2表达,值得推广。Objective:To explore the effect of Jianpi Qingdu Decoction combined with chemotherapy on the levels of immunoglobulin(Ig),toxic and side effects,cyclooxygenase-2(COX-2)and prostaglandin E_(2)(PGE_(2))for patients with gastric cancer.Methods:86 patients with gastric cancer admitted to Huai’an cancer hospital from January 2,016 to January 2,020 were selected and randomly divided into a controlled group and an observation group,43 cases in each group.The controlled group was treated with chemotherapy,and the observation group was treated with Jianpi Qingdu Decoction on the basis of the controlled group.The clinical efficacy and TCM symptom scores,health status,serum Ig and COX-2,PGE_(2)levels and the occurrence of toxic and side effects were compared between the two groups of patients before and after treatment.Results:The disease control rate of the observation group was significantly higher than that of the controlled group(P<0.05).The observation group’s TCM symptom score and serum COX-2,PGE_(2)levels,platelet decline,peripheral nerve abnormalities,alanine aminotransferase(ALT)elevation,white blood cell decline,and serum creatinine elevation ratio were significantly lower than those of the controlled group,Karnofsky functional status(KPS)score and serum IgA the levels of IgG and IgM were significantly higher than those of the controlled group(P<0.05).Conclusion:Jianpi Qingdu Decoction combined with chemotherapy has the effect of increasing efficacy and reducing toxicity in the treatment of gastric cancer,which can significantly enhance the immunity of patients,improve the health status,and inhibit the expression of COX-2 and PGE_(2),it is worth promoting.
