白内障术后排便憋气致虹膜疝5年1例

摘　　要：1例主诉为右眼异物感5年的患者就诊于浙江省医疗健康集团衢州医院(浙江衢化医院)眼科。既往有高血压病史,5年前行白内障手术,术后初期恢复良好。术后2周排便憋气后出现异物感、眼红、流泪症状,当地复诊后予以眼药水滴眼治疗,后眼红流泪消失,异物感仍存在,后续未予进一步治疗。在本次就诊中发现患者角膜原主切口有虹膜疝,覆盖膜样物质。诊断:右眼虹膜疝,右眼白内障术后,高血压。经评估无手术禁忌,行坏死虹膜组织切除和瞳孔重建手术。术后门诊随访,患者右眼症状改善,视力提高,虹膜没有再次脱出。A patient who complained of 5-year foreign body sensation in the right eye vis ited the Ophthalmology Department of Zhejiang Medical and Health Group Quzhou Hospital(Zhejiang Quhua Hospital).The patient had a previous history of hypertension and underwent cataract surgery 5 years ago,with good rehabilitation in the early postoperative period.However,two weeks after the surgery,the patient developed foreign body sensation,eye redness and tearing after suffocation for defecation.After a re-visit locally,eye redness and tearing resolved due to the application of eyedrops,while foreign body sensation remained,no further treatment received.During this visit,an iris hernia in the original primary incision of the cornea was found,covering a membrane-like substance.The patient was then diagnosed as:iris hernia in the right eye,with a treatment history of cataract surgery in the right eye and a hypertension history.Necrotic iris tissue excision and pupil reconstruction were performed after evaluation of no contraindications to surgery.At postoperative follow-up at the outpatient,the patient’s right eye symptoms resolved,visual acuity improved,with no iris detachi ng again.
