摘 要:目的调查邯郸市主城区1~6年级学生眼屈光状态及生物学参数,探讨晶状体屈光力和轴径比等对屈光状态的影响,并评估其对屈光发育的监测作用。方法采用横断面调查设计。抽取邯郸市主城区10所小学2508名小学生,通过光学生物测量仪(IOL-Master)和电脑验光仪获得眼轴(Axis length,AL)、角膜屈光力、前房深度(Anterior chamber depth,ACD)和等效球镜(spherical equivalent,SE),并计算晶状体屈光力(Lens refractive power,LP)、眼轴长度/角膜曲率半径(AL/CR)。按年级分为6组,并按屈光状态分为近视组、正视组和远视组。采用独立样本t检验、单因素方差分析及线性回归分析等方法分析不同年龄,不同屈光组之间各参数的关系及各生物学参数对屈光状态的影响。结果1~6年级组SE(F=48.642,P<0.05)及LP(F=287,P<0.05)随年龄增长逐渐变小,AL/CR(F=64.523,P<0.05)、AL(F=62.468,P<0.05)、ACD(F=3.876,P<0.05)随年级增长逐渐增大,角膜曲率(F=2.631,P=0.058)变化较稳定。近视组和远视组,LP与SE均呈正相关(r=0.489、0.862,均P<0.05),AL/CR和AL与SE均负关性(r=-0.743、-0.468、-0.481、-0.509,均P<0.05)。结论邯郸市主城区1~6年级学生眼球生长发育过程中LP和AL变化较明显,LP、AL/CR、AL均与SE存在相关性,LP、AL/CR是监测近视发生、发展的重要指标。Objective To investigate the refractive status and biological parameters of students in grade one to grade six in Handan city,explore the influence of lens refractive power and axial length/corneal curvature radius(AL/CR)on refractive status,and evaluate its monitoring effect on refractive development.Methods Cross-sectional survey design was adopted.2508 primary school students from 10 schools in Handan city were selected,and the axial length(Al),corneal refractive power,Anterior chamber depth(ACD)and spherical equivalent(SE)were obtained by optical bio-meter(10L-Master)and computer refractometer.and the lens refractive power(LP)were calculated.According to grade,they were divided into 6 groups,and according to refractive status,they were divided into myopia group,emmetropia group and hyperopia group.Independent sample t test,one-way ANOVA and linear regression were used to analyze the relationship.The relationships of above parameters among different age groups and among different refractive groups and their effect on refractive status were analyzed.Results SE(F=48.642,P<0.05)and LP(F=287,P<0.05)in the first to sixth grade groups gradually decreased with age,AL/CR(F=64.523,P<0.05),AL(F=62.468,P<0.05)and ACD(F=3.876,P<0.05)increased gradually with the increase of grade,corneal curvature(F=2.631,P=0.058)was stable.In myopia group and hyperopia group,LP was positively correlated with SE(r=0.489 and 0.862,all P<0.05),while AL/CR and Al were negatively correlated with SE(r=-0.743,-0.468,-0.481 and-0.509,all P<0.05).Conclusions The changes of LP and AL in the eye growth and development of students from grade one to grade six in Handan city are obvious.LP AL/CR and AL are all correlated with SE.LP and AL/CR are important indicators to monitor the occurrence and development of myopia.
