摘　　要：目的探讨Spot双目视力筛查仪在儿童屈光筛查中的临床应用效果。方法将2021年5月至2022年5月在南通市妇幼保健院眼科门诊就诊的儿童分为两个年龄段,3~6岁和>6岁,每个年龄段150例(300眼),Spot双目视力筛查仪屈光筛查和睫状肌麻痹检影验光,四格表计算Spot双目视力筛查仪的判读结果灵敏度、特异度和准确度。秩和检验计算两者测量数值差异,线性回归分析两者相关性。Bland-Altman方法评估两者一致性。结果Spot双目视力筛查仪在3~6岁组灵敏度、特异度和准确度分别为74.32%、94.74%、84.67%,>6岁组分别为78.23%、80.77%、78.67%。3~6岁组Spot双目视力筛查仪较检影验光低估了球镜值(Z=-13.735,P<0.01)及等效球镜(Z=-14.232,P<0.01),高估了柱镜值(Z=-8.562,P<0.01)。在>6岁组Spot双目视力筛查仪较检影验光低估了等效球镜(Z=-4.172,P<0.01),高估了球镜值(Z=-4.051,P<0.01)及柱镜值(Z=-13.193,P<0.01)。3~6岁组两者球镜、柱镜及等效球镜相关系数分别为0.794、0.867、0.653;>6岁组两者相关系数分别为0.928、0.566、0.917,均有明显相关性(P<0.01)。3~6岁组球镜、柱镜、等效球镜的95%一致性界限分别包括了94.33%、95.67%、94.00%的受试眼,>6岁组分别为95.00%、93.67%、95.00%。结论Spot双目视力筛查仪具有较高的灵敏度、特异度和准确度,与睫状肌麻痹检影验光结果虽有差异但有高度正相关性,可适用于儿童的屈光筛查,在临床中有较高使用价值。Objective To explore the application effect of Spot vision screener in children's refractive screening and whether it can be used in clinic.Methods Children who visited the ophthalmic outpatient department of Maternal and Child Health Care Hospital of Nantong from May 2021 to May 2022 were divided into two age groups:3~6 years old and>6 years old,150 cases(300 eyes)in each group.They underwent Spot vision screener for refractive screening and cycloplegic retinoscopy.Four grid table was used to calculate the sensitivity,specificity and accuracy of the interpretation results of Spot vision screener.Rank sum test was used to calculate the difference between Spot vision screener and cycloplegic retinoscopy,and regression was used to analyze the correlation between them.Bland-Altman method was used to evaluate the consistency between them.Results The sensitivity,specificity and accuracy of Spot vision screener in 3~6 years old group were 74.32%,94.74%and 84.67%,respectively,and those in>6 years old group were 78.23%,80.77%and 78.67%,respectively.In the 3~6 years old group,the Spot vision screener underestimated spherical power(Z=-13.735,P<0.01)and spherical equivalent(Z=-14.232,P<0.01)and overestimated cylindrical power(Z=-8.562,P<0.01)compared with the cycloplegic retinoscopy.In>6 years old group,the Spot vision screener underestimated spherical equivalent(Z=-4.172,P<0.01)and overestimated spherical power(Z=-4.051,P<0.01)and cylindrical power(Z=-13.193,P<0.01)compared with the cycloplegic retinoscopy.The correlation coefficients of sphere,cylinder and spherical equivalent in 3~6 years old group were 0.794,0.867 and 0.653 respectively,in>6 years old group were 0.928,0.566 and 0.917 respectively,which were statistically correlated(P<0.01).In 3~6 years old group,the 95%limit of agreement cover rates between the Spot vision screener and cycloplegic retinoscopy in sphere,cylinder and spherical equivalent were 94.33%、95.67%、94.00%,respectively,and in>6 years old group were 95.00%、93.67%、95.00%,respectively.Conclusi
