矫正胎龄45周以上早产儿视网膜病变眼底特征

摘　　要：目的观察早产儿视网膜病变(ROP)在较大矫正胎龄阶段的眼底病变程度及特征。方法回顾性分析2019年1月1日至2022年6月30日矫正胎龄大于45周行荧光素眼底血管造影(FFA)检查的ROP患儿眼底图像及造影检查结果。结果纳入ROP患儿20例(40眼)。眼底图像中周边视网膜血管自然终止的5眼(12.5%),其中3眼(60.0%)FFA显示视网膜末梢血管存在点状荧光渗漏;嵴样病变33眼(82.5%),其中31眼(93.9%)FFA显示不同程度的片状荧光渗漏;新生血管纤维增殖的2眼(5.0%),FFA均显示严重荧光渗漏。结论矫正胎龄45周以上ROP患儿必要时需行FFA检查;若FFA显示严重荧光渗漏,需积极手术干预。Objective To observe the degree and characteristics of fundus lesions in retinopathy of prematurity(ROP)in the larger corrected gestational age.Methods A retrospective analysis of 40 eyes of 20 ROP cases with corrected gestational ages over 45 weeks from January 1,2019 to June 30,2022.Fundus images and Fundus fluorescence angiography(FFA)outcomes were recorded.Result In fundus images,5 eyes(12.5%)showed spontaneous termination of peripheral retinal blood vessels and 3 of them(60.0%)FFA showed punctuate fluorescence leakage in peripheral retinal blood vessels.33 eyes(82.5%)show wide ridge in fundus images and 31 of them(93.9%)FFA showed varying degrees of flaky fluorescence leakage.2 eyes(5.0%)showed neovascular fiber proliferation and FFA showed severe fluorescence leakage.Conclusion FFA examination is necessary for ROP infants with corrected gestational age over 45weeks.If FFA shows severe fluorescence leakage,surgical intervention is indicated.
