儿童先天性上睑下垂合并先天性睑内翻倒睫的临床特征及手术治疗

摘　　要：目的观察CP合并CET的临床特征,探讨手术治疗方式及疗效。方法回顾性分析2013年1月至2021年9月接受手术治疗的209例CP患儿临床资料,单纯CP 93例为对照组,CP合并CET 116例为观察组。比较两组的症状和体征,观察手术效果及术后并发症。结果两组最常见的症状为眨眼/揉眼,均出现不同程度的仰头视物、眼红、畏光、流泪症状。以上症状发生率差异均无统计学意义(P>0.05)。两组角膜损伤以轻中度为主,CP以中重度为主。两组不同程度角膜损伤发生率及不同程度CP发生率差异无统计学意义(P>0.05)。视力/屈光异常发生率差异有统计学意义(P=0.042)。观察组CET手术满意95.6%,基本满意4.4%,无不满意。观察组CP手术效果满意72.7%,基本满意19.9%,不满意7.5%。术后4例(4眼)倒睫伤口拆线时裂开,7例(7眼)出现结膜脱垂,2例(2眼)上睑弧度不佳,成角畸形。结论CP合并CET患儿一般下睑CET程度以轻中度为主,无特异性症状,视力/屈光异常发生率较单纯CP更高,CET和CP分次手术矫正治疗效果理想,并发症少。Objective To investigate the clinical features of congenital ptosis coexisting congenital entropion and trichiasis in children.To explore the surgical treatment and efficacy.Methods The clinical data of 209 cases with CP were retrospectively analyzed,93 cases with simple CP as the control group and 116 cases CP coexisting CET as the observation group from January 2013 to September 2021.The symptoms and signs of were summarized.The surgical effect and postoperative complications were observed.Results The most common symptom was blinking/eye rubbing.The other symptoms included head posture,redness,photophobia,and tears.There was no statistical difference in the incidence of these symptoms between the two groups(P>0.05).In the two groups,the degree of corneal injury was both mild and moderate,and degree of CP was both moderate and severe.There was no statistical difference in the incidence of different degrees corneal injury and CP(P>0.05).The incidence of visual acuity/refractive abnormalities between the two groups was statistically significant(P=0.042).In the observation group,the CET effect 95.6%were satisfied,and 4.4%were basically satisfied.The CP effect 72.7%were satisfied,19.9%were basically satisfied,and 7.5%were dissatisfied.Postoperative complications including 4 cases(4 eyes)split the lower eyelid wound,7 cases(7 eyes)had conjunctival prolapse,and two cases(2 eyes)had the upper eyelid angulation deformity.Conclusions In children with CP coexisting CET,the degree of CET in the lower eyelid is mainly mild and moderate,no specific symptoms,but the incidence of vision/refraction is higher than that of simple CP.The surgical effect is totally good,with fewer complications.
