活动基托式快速扩弓器适配压膜合垫+前方牵引器矫治替牙早期骨性反合的临床疗效

摘　　要：目的评估上颌活动基托式快速扩弓矫治器配合压膜合垫+前方牵引治疗替牙早期安氏Ⅲ类骨性错合的临床疗效。方法选取20例上颌骨发育不足伴牙弓宽度不调的安氏Ⅲ类替牙早期患者,采用上颌活动基托式快速扩弓矫治器配合压膜合垫+前方牵引进行矫治,比较患者治疗前后X线头影测量标志点的变化。结果治疗后所有患者前、后牙反合均解除,上颌凹面型明显改善;头影测量分析显示治疗后SNA、ANB、上下前牙覆盖及上唇突度平均增加分别为(2.98±0.59)°、(3.22±0.82)°、(3.78±0.53)mm、(3.03±0.45)mm,A点明显前移,有显著性差异(P<0.001)。结论上颌活动基托式快速扩弓矫治器配合压膜合垫+前方牵引可有效治疗替牙早期上颌骨发育不足伴横向宽度不调安氏Ⅲ类骨性错合,是一种简单有效的方法。Objective To study the clinical effects of rapid maxillary expansion with removable split acrylic palate and the maxillary protraction on patients with skeletal Class Ⅲ malocclusion in early mixed dentition.Methods Twenty patients with skeletal Class Ⅲ malocclusion and transverse deficiency were selected.All patients in early mixed dentition were treated by maxillary protraction combined with rapid maxillary expansion with removable split acrylic palate.Cephalometric analysis was carried out before and after treatment.Results After maxillary protraction,the maxilla was displaced anteriorly,point A protracted obviously,the anterior and posterior crossbite were relieved,the Class Ⅲ concave profile became more balanced,with the upper lip area becoming more marked.Significant improvement rjet(3.78±0.53)mm and ULP(3.03±0.45)mm.Conclusion These findings indicate that the maxillary protraction with occlusal pad and rapid maxillary expansion with removable split acrylic palate are effective for correcting skeletal Class Ⅲ malocclusions caused by maxillary retrognathism in the early mixed dentition.
