精准消融理念在治疗合并严重肝硬化微小肝癌中的应用

摘　　要：精准消融是以患者的生存期和生存质量为根本出发点,借助多种影像学引导,以安全、高效、极微创为目标的全流程、全要素的肿瘤消融实践规范。合并严重肝硬化的微小肝癌由于其特殊的疾病特点,术前影像学引导定位困难、肝功能较差、储备体积小、术中消融针偏离肝癌病灶、术后无瘤生存期较短等严重影响射频消融的应用。精准医学理念已迅速渗透至肿瘤医学等诸多临床医学领域,其理念在合并严重肝硬化微小肝癌的诊断与治疗中起到了重要作用,即以完全消融肝癌病灶MVI体积为核心,通过定性、定位、定构技术,可以提高合并严重肝硬化微小肝癌患者消融前评估及消融过程中的确定性,不断延长其生存期及提高生存质量。文章深入阐述精准消融的核心、内涵,以及在治疗合并严重肝硬化微小肝癌中的应用实践。Precise ablation is a full-process,all-factor tumor ablation practice with the basic starting point of the patient's survival period and quality of life,with the help of a variety of imaging guidance,and the goal of safety efficiency and extremely minimally invasive.Due to its special disease characteristics,the application of radiofrequency ablation is seriously affected by the difficulty of preoperative imaging guidance,poor liver function,small reserve volume,deviation of ablation needle from the focus of liver cancer during operation,and short tumor-free survival period after operation.The concept of precision medicine has rapidly penetrated into many clinical medical fields such as tumor medicine.Its concept has played an important role in the diagnosis and treatment of microhepatocellular carcinoma with severe cirrhosis.It takes the complete ablation of the MVI volume of the liver cancer focus as the core,and through qualitative,positioning,and structural techniques,it can improve the pre-ablation evaluation and the certainty in the ablation process of patients with microhepatocellular carcinoma with severe cirrhosis,continuously extend its survival period and improve its quality of life.The author deeply expounds the core and connotation of precision ablation and its application in the treatment of microhepatocellular carcinoma with severe cirrhosis.
