子宫肌瘤患者子宫动脉血流动力学指标与子宫体积关系及对肌瘤变性诊断效能研究

摘　　要：目的:探讨子宫肌瘤患者子宫动脉血流动力学指标与子宫体积的关系及对肌瘤变性的诊断效能。方法:选择行手术治疗的185例子宫肌瘤患者作为观察组,另选同期的185例健康体检女性作为对照组。检测两组子宫动脉血流动力学指标[子宫动脉搏动指数(PI)、阻力指数(RI)、收缩期峰值流速(PSV)、舒张末期流速(EDV)、舒张期最小血流速度(Vmin)],使用Pearson相关性分析子宫动脉血流动力学指标与子宫肌瘤患者子宫体积的关系;观察肌瘤变性的发生情况,以受试者工作特征(ROC)曲线下面积(AUC)评价子宫动脉血流动力学指标对肌瘤变性的诊断效能。结果:观察组子宫动脉PI、PSV、EDV、Vmin大于对照组,RI小于对照组(均P<0.05);经Pearson相关性分析,子宫肌瘤患者子宫体积与子宫动脉PI、PSV、EDV、Vmin呈正相关(均P<0.05),与RI呈负相关(P<0.05);在观察组185例子宫肌瘤患者中,经术后病理活检确诊肌瘤变性20例;肌瘤变性组子宫动脉RI大于非肌瘤变性组,PSV小于非肌瘤变性组(均P<0.05);子宫动脉RI联合PSV诊断子宫肌瘤患者发生肌瘤变性的AUC为0.910。结论:子宫肌瘤患者存在子宫动脉流速增大、阻力减小,与子宫体积增大密切相关,子宫动脉RI联合PSV诊断肌瘤变性的效能较好,值得临床予以重视。Objective:To analyze the relationship between uterine arterial hemodynamics and uterine volume in patients with uterine fibroids and the diagnostic efficacy of fibroid degeneration.Methods:A total of 185 patients with uterine fibroids who underwent surgical were selected as the observation group,and 185 healthy women who underwent physical examination during the same period were selected as the control group.The hemodynamic parameters of uterine arteries[uterine artery Pulsatility Index(PI),Resistance Index(RI),peak systolic flow velocity(PSV),end-diastolic flow velocity(EDV),and minimum diastolic flow velocity(Vmin)]were detected in the two groups.Pearson correlation was used to analyze the relationship between the hemodynamic parameters of uterine arteries and uterine volume in patients with uterine fibroids.The incidence of fibroid degeneration was observed,and the area under receiver operating characteristic(ROC)curve(AUC)was used to evaluate the diagnostic efficacy of uterine arterial hemodynamics indexes for fibroid degeneration.Results:The PI,PSV,EDV and Vmin of uterine artery in the observation group were higher than those in the control group,and RI was lower than that in the control group(all P<0.05).Pearson correlation analysis showed that uterine volume in patients with uterine fibroids was positively correlated with uterine artery PI,PSV,EDV and Vmin,and negatively correlated with RI(all P<0.05).In the observation group of 185 patients with uterine fibroids,20 cases were confirmed as fibroid degeneration by postoperative pathological biopsy.The uterine artery RI in the fibroid degeneration group was higher than that in the non-fibroid degeneration group,and the PSV was lower than that in the non-fibroid degeneration group(all P<0.05).The AUC of uterine artery RI combined with PSV in the diagnosis of fibroid degeneration in patients with uterine fibroids was 0.910.Conclusion:Increased uterine arterial flow velocity and decreased resistance are associated with increased uterine volume in patients with
