摘  要:目的探讨相干光层析血管成像术(OCTA)对孔源性视网膜脱离(RRD)复位术后黄斑区血流状况的诊断作用。方法回顾性病例研究。选取2019年1月至2021年9月因单眼RRD在我院成功接受玻璃体切除手术治疗的43例患者为研究对象,以视网膜复位满意后3个月的患眼为研究组,对侧健眼为对照组,对两组行OCTA检测,获取黄斑中心凹厚度(CMT)、黄斑中心凹无血管区(FAZ)面积、周长、黄斑中心凹及旁中心凹处浅层毛细血管层(SCP)血流密度、深层毛细血管层(DCP)血流密度及总体血流密度数据,并对结果进行对比分析。结果两组眼轴长度、眼压,无明显差异(P>0.05)。研究组BCVA、CMT低于对照组;而FAZ面积及周长均高于对照组,两组间比较差异有统计学意义(P<0.05)。研究组黄斑中心凹处SCP、DCP血流密度、旁中心凹处SCP血流密度及总体血流密度均低于对照组,有显著性差异(P<0.05);而两组旁中心凹处DCP血流密度无明显差异(P>0.05)。结论RRD离患者在接受玻璃体切除视网膜复位术后,黄斑区血流有明显变化,OCTA检查可检测患者术后视网膜结构、血流变化,具有一定临床价值。Objective To investigate the diagnostic effect of optical coherence tomography angiography(OCTA)on macular blood flow after rhegmatogenous retinal detachment reduction.Methods This retrospective case series enrolled 43 patients with rhegmatogenous retinal detachment(RRD)who underwent vitrectomy in our hospital from January 2019 to September 2021.The eyes at three months after the operation were taken as the study group and the contralateral healthy eyes were taken as the control group.The results of OCTA were compared and analyzed between the two groups.Results There was no significant difference in axial length and intraocular pressure between the two groups(P>0.05).The best corrected visual acuity(BCVA)and central macular thickness(CMT)in the study group was lower than that in the control group(P<0.05).The area and perimeter of foveal avascular zone(FAZ)in the study group was higher than that in the control group(P<0.05).The blood flow density of superficial retinal capillary plexuses(SCP)and deep retinal capillary plexues(DCP)in foveal area and the total blood flow density in macular area was significantly lower in the study group than that in the control group(P<0.05).There was no significant difference in blood flow density of paralaminar fovea of DCP between the two groups(P>0.05).Conclusions In patients with RRD,there are significant changes in macular blood flow after vitrectomy and retinal reduction.The OCTA test can detect the changes of retinal structure and function after the operation,which has a certain clinical value.
