两种不同切口下的白内障超声乳化术对角膜内皮细胞的影响

摘　　要：目的评价透明角膜切口下的白内障超声乳化术与巩膜隧道切口下的白内障超声乳化术对角膜内皮细胞的影响。方法前瞻性病例对照研究。选取符合纳入标准的40例(40只眼)年龄相关性白内障患者随机分成两组,A组20只眼行透明角膜切口的白内障超声乳化手术联合人工晶状体植入术;B组20只眼行巩膜隧道切口的超声乳化术联合人工晶状体植入术。并对术前、术后1周、1个月的角膜内皮细胞数据进行比较。结果将两组内术后1周及术后1个月角膜内皮细胞密度与术前相比,两组术后角膜内皮细胞密度均有下降,差异有统计学意义(P<0.05)。将两组间术后与术前角膜内皮细胞密度差值相比,巩膜隧道切口组低于透明角膜切口组,差异有统计学意义(P<0.05)。结论巩膜隧道切口的白内障超声乳化术对角膜内皮细胞的损伤较透明角膜切口的超声乳化术更轻。Objective To compare the effects on corneal endothelial cell(CEC)after phacoemulsification(Phaco)with scleral tunnel and clear corneal incisions.Methods A prospective case control study.A total of 40 patients(40 eyes)who met the criteria were randomly divided into two groups.Cataract phacoemulsification and intraocular lens implantation surgery were carried out with scleral tunnel incision(A group,20 eyes)and clear corneal incision(B group,20 eyes).The changes of CEC density were compared before the operation,at 1 week and 1 month after the operation.Results The postoperative CEC density was significantly lower as compared to the preoperative in both groups(P<0.05).The difference of CEC density before the operation,at 1 week and 1 month after the operation was smaller in the A group as compared with the B group(P<0.05).Conclusions The damage to corneal endothelial cell of phacoemulsification under scleral tunnel incision was less than phacoemulsification under clear corneal incision.
