摘　　要：目的探讨iRoot SP辅助单尖充填技术一次性根管治疗牙隐裂伴牙髓炎的临床效果。方法选取2019年6月至2021年10月于本院就诊的70例(70颗牙)牙隐裂伴牙髓炎患者作为研究对象,采用随机数字表法将其分为对照组与试验组,各35例(35颗牙)。对照组采用常规多次根管治疗结合热牙胶垂直加压充填技术,试验组采用iRoot SP辅助单尖充填技术一次性根管治疗。比较两组的术后24 h疼痛情况、根管充填时间、临床疗效、咬合力、松动度及牙龈指数。结果试验组术后24 h的总疼痛率为5.71%,低于对照组的25.71%,差异具有统计学意义(P<0.05)。试验组的根管充填时间短于对照组,差异具有统计学意义(P<0.05)。两组的治疗总有效率比较,差异无统计学意义(P>0.05)。治疗后7 d,两组的咬合力均升高,松动度及牙龈指数评分均降低,差异具有统计学意义(P<0.05);治疗后7 d,试验组的咬合力高于对照组,牙龈指数评分低于对照组,差异具有统计学意义(P<0.05)。结论iRoot SP辅助单尖充填技术一次性根管治疗牙隐裂伴牙髓炎的临床效果较好,可缩短根管填充时间,减轻术后疼痛程度,改善牙齿咬合力、松动度及牙龈指数。Objective To explore the clinical effect of iRoot SP assisted single tip filling technique in one-time root canal treatment of cracked teeth with pulpitis.Methods A total of 70 patients(70 teeth)with cracked teeth and pulpitis who were treated in our hospital from June 2019 to October 2021 were selected as the research objects.The patients were divided into control group and experimental group by random number table method,with 35 cases(35 teeth)in each group.The control group was treated with conventional multiple root canal therapy combined with hot gutta-percha vertical compression filling technique,and the experimental group was treated with iRoot SP assisted single tip filling technique in one-time root canal treatment.The postoperative 24 h pain,root canal filling time,clinical efficacy,bite force,looseness and gingival index were compared between the two groups.Results The total postoperative 24 h pain rate of the experimental group was 5.71%,which was lower than 25.71%of control group,and the difference was statistically significant(P<0.05).The root canal filling time of the experimental group was shorter than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).At 7 d after treatment,the bite force of the two groups increased,the looseness and gingival index scores decreased,and the differences were statistically significant(P<0.05);at 7 d after treatment,the bite force of the experimental group was higher than that of the control group,the gingival index score was lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion The iRoot SP assisted single tip filling technique in one-time root canal treatment of cracked teeth with pulpitis has good clinical effect.It can shorten the root canal filling time,reduce the degree of postoperative pain,improve the bite force,looseness of teeth and gingival index.
