OCTA量化分析BRVO黄斑无血流区面积及微血管改变

摘　　要：目的 应用光学相关断层扫描血流成像(optical coherence tomography angiography, OCTA)对比观察不同缺血分型的视网膜分支静脉阻塞(branch retinal vein occlusion, BRVO)患者的黄斑无血流区(macular non-flow, MNF)面积及其他微血管参数变化,并分析MNF面积与其余各项参数的相关性。方法 采用回顾性研究方法,选取2020年8月~2021年12月于徐州医科大学附属医院就诊,经检查后确诊为BRVO患者45例(45只患眼),其中非缺血型25例(25只眼),缺血型20例(20只眼),OCTA检查分析获得黄斑中心6mm×6mm大小范围内MNF面积、视网膜浅层毛细血管丛血流密度(superficial vascular density, SVD)、深层毛细血管丛血流密度(deep vascular density, DVD)、中心凹无血管区(foveal avascular zone, FAZ)面积、FD-300等参数,比较非缺血组和缺血组的上述指标,并分析MNF面积与其余各项参数的相关性。结果 与非缺血组比较,缺血组MNF面积明显增加,SVD、DVD、FD-300均明显下降,差异均有统计学意义(P均<0.05),FAZ面积无明显变化。MNF面积与SVD、DVD、FD-300均呈负相关(r分别为-0.678、-0.630、-0.480,P均<0.05),而与FAZ面积无明显相关性。结论 MNF可作为一项新的OCTA指标来评估BRVO黄斑区视网膜的缺血状态;BRVO两组间比较,DVD相比SVD下降更为明显,提示深层毛细血管丛的毛细血管损伤对于视网膜缺血更为敏感。Objective To observe changes in macular non-flow(MNF)area and other microvascular parameters in patients with different ischemic types of branch retinal vein occlusion(BRVO)acquired by OCTA,and analyze the correlation between MNF area and other various parameters.Methods In this retrospective study,45 patients(45 affected eyes)diagnosed with BRVO were selected from the Affiliated Hospital of Xuzhou Medical University from August 2020 to December 2021,including 25 patients(25 eyes)with non-ischemic and 20 patients(20 eyes)with ischemic.The area of MNF,superficial vascular density(SVD),deep vascular density(DVD),area of foveal avascular zone(FAZ)and FD-300 within the size range of 6mm×6mm in the macular center acquired by OCTA.The above indexes were compared between the non-ischemia group and the ischemia group,and the correlation between the area of MNF and other parameters was analyzed.Results Compared with the non-ischemic group,the area of MNF in the ischemic group increased significantly,the SVD,DVD and FD-300decreased significantly,with statistical significance(all P<0.05),and the area of FAZ had not change significantly.The area of MNF was negatively correlated with SVD,DVD and FD-300(r were-0.678,-0.630,-0.480,all P<0.05),but there was no significant correlation with the area of FAZ.Conclusion MNF can be used as a new OCTA index to evaluate the ischemic state of retina in BRVO macular region;Compared with the two groups of BRVO,the decrease of DVD was more significantly than SVD,suggesting that the capillary injury of deep capillary plexus is more sensitive to retinal ischemia.
