早期经口进食治疗轻症及中度重症急性胰腺炎患者的效果观察

摘　　要：目的:探讨早期经口进食治疗轻症急性胰腺炎(MAP)及中度重症急性胰腺炎(MSAP)患者的效果。方法:选取2017年1月—2019年12月北京市石景山医院消化科收治的68例MAP及MSAP患者为观察对象,根据营养方式不同分为对照组与观察组。对照组(33例)给予全胃肠外营养,观察组(35例)给予肠内营养(EN)。比较两组患者临床指标、实验室指标水平、并发症发生情况。结果:观察组禁食时间、住院时间短于对照组,住院费用低于对照组,差异有统计学意义(P<0.01)。观察组血清淀粉酶、血清脂肪酶、C反应蛋白降低值高于对照组,差异有统计学意义(P<0.01)。结论:MAP和MSAP患者早期行EN,不会加重病情,有助于缩短住院时间、减少住院费用,具有较高的安全性及经济效益。Objective:To investigate the effect of early oral feeding in the treatment of patients with mild acute pancreatitis(MAP)and moderate severe acute pancreatitis(MSAP).Methods:A total of 68 MAP and MSAP patients admitted to Department of Gastroenterology in Beijing Shijingshan Hospital from January 2017 to December 2019 were collected as the study subjects.The patients were divided into observation group and control group according to the different nutrition approaches.The control group(33 cases)was given total parenteral nutrition and the observation group(35 cases)was given enteral nutrition(EN).The clinical indexes,laboratory index levels and complication occurrence were compared between the two groups.Results:The fasting time and length of hospital stay in the observation group were shorter than those in the control group,and the hospitalization cost in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.01).The serum amylase,serum lipase and C-reactive protein decreases in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.01).Conclusion:Early EN in MAP and MSAP patients will not aggravate the disease,it helps to shorten the length of hospital stay,reduce the cost of hospitalization,with higher safety and economic benefits.
