TCT联合HPV筛查转诊阴道镜活检在宫颈癌筛查中的价值分析

摘　　要：目的研究液基薄层液基细胞学(TCT)联合人乳头瘤病毒(HPV)筛查转诊阴道镜活检对宫颈癌筛查的价值。方法选取460例行宫颈癌筛查的女性作为研究对象,先行TCT联合HPV筛查,然后将筛查阳性者转诊阴道镜检查,并将最终病理诊断结果作为金标准,比较不同检查方法的宫颈癌检出情况,分析TCT联合HPV筛查转诊阴道镜活检对宫颈癌的诊断价值。结果460例患者经病理诊断检出宫颈癌57例(12.39%),经TCT联合HPV检出62例(13.48%),经TCT联合HPV转诊阴道镜活检检出59例(12.83%),不同检查方法的宫颈癌检出率比较差异无统计学意义(P>0.05)。以病理诊断结果作为金标准,TCT联合HPV筛查转诊阴道镜活检的阳性预测值、阴性预测值、敏感度、特异度、准确度、约登指数、误诊率、漏诊率分别为94.92%(56/59)、99.75%(400/401)、98.25%(56/57)、99.26%(400/403)、99.13%(456/460)、0.9751、0.74%(3/403)、1.75%(1/57)。结论TCT联合HPV筛查转诊阴道镜活检在宫颈癌的筛查中具有较高的筛查质量,可以为宫颈癌的筛查提供科学的依据,值得推荐。Objective To study the value of thinprep cytologic test(TCT),human papilloma virus(HPV)screening combined with colposcopy biopsy in cervical cancer screening.Methods A total of 460 women undergoing cervical cancer screening were selected as subjects,and the positive patients were first screened by TCT combined with HPV,and then referred to colposcopy.The final pathological diagnosis results were used as the gold standard to compare the detection of cervical cancer by different screening methods,and analyze the diagnostic value of TCT,HPV screening combined with colposcopy biopsy for cervical cancer.Results In 460 patients,57 cases(12.39%)of cervical cancer were detected by pathological diagnosis,62 cases(13.48%)by TCT combined with HPV,and 59 cases(12.83%)by TCT,HPV screening combined with colposcopy biopsy.There was no statistically significant difference in the detection rate of cervical cancer among different screening methods(P>0.05).Taking the pathological diagnosis results as the gold standard,the positive predictive value,negative predictive value,sensitivity,specificity,accuracy,Youden's index,misdiagnosis rate and missed diagnosis rate of TCT,HPV screening combined with colposcopy biopsy were 94.92%(56/59),99.75%(400/401),98.25%(56/57),99.26%(400/403),99.13%(456/460),0.9751,0.74%(3/403)and 1.75%(1/57).Conclusion TCT,HPV screening combined with colposcopy biopsy has a high screening quality in cervical cancer screening,and can provide scientific basis for cervical cancer screening,which is worthy of recommendation.
