体外圈套器牵引辅助内镜黏膜下剥离术在老年消化道早癌治疗中的应用研究

摘　　要：目的研究体外圈套器牵引辅助内镜黏膜下剥离术(ESD)在老年消化道早癌治疗中的应用效果及安全性。方法选择2020年1月—2021年12月于龙岩市第二医院治疗的84例老年消化道早癌患者,按随机数字表将其分为两组,各42例。观察组ESD术中采用体外圈套器牵引辅助治疗,对照组采用传统ESD治疗。比较两组术中指标、术后恢复指标、病灶完整切除率及并发症。结果观察组术中出血量低于对照组,手术时间短于对照组(P<0.001)。观察组住院时间短于对照组,VAS评分低于对照组(P<0.001)。观察组病灶完整切除率为100.00%(42/42),对照组病灶完整切除率为95.24%(40/42),两组比较差异无统计学意义(χ^(2)=0.512,P>0.05)。观察组并发症总发生率为4.76%,低于对照组的19.05%(P<0.05)。结论老年消化道早癌患者ESD术中采用体外圈套器牵引辅助治疗可将病灶完整切除,手术耗时短且出血少,疼痛轻微,且并发症少,临床应用安全可靠。Objective To study the effect and safety of endoscopic submucosal dissection(ESD)assisted by external loop traction in the treatment of early gastrointestinal cancer in the elderly.Methods 84 elderly patients with early gastrointestinal cancer treated in The Second Hospital of Longyan from January 2020 to December 2021 were randomly divided into two groups(observation group and control group),with 42 cases in each group.The observation group were treated with ESD assisted by external loop traction,and the control group were treated with traditional ESD.And then,intraoperative indexes,postoperative recovery indexes,complete resection rates and complications were compared between the two groups.Results The amount of intraoperative bleeding in the observation group was lower than that in the control group,and the operation time was shorter than that in the control group(P<0.001).The hospital stay of the observation group was shorter than that of the control group,and the VAS score was lower than that of the control group(P<0.001).The complete resection rate of the observation group was 100.00%(42/42),and that of the control group was 95.24%(40/42),and difference between the two groups was not statistically significant(χ^(2)=0.512,P>0.05).The total incidence of complications of the observation group was 4.76%,which was lower than that of the control group(19.05%)(P<0.05).Conclusion ESD assisted by external loop traction in senile patients with early gastrointestinal cancer can completely remove the focus.It takes short operation time with less bleeding,mild pain,and few complications.The clinical application is safe and reliable.
