消化内镜下和谐夹与金属钛夹止血治疗上消化道出血患者的效果比较

摘　　要：目的:比较消化内镜下可旋转重复开闭软组织夹(和谐夹)与金属钛夹止血治疗上消化道出血患者的效果。方法:选取2019年1月至2021年11月该院收治的61例上消化道出血患者进行前瞻性研究,按照随机数字表法分为对照组(n=30)和观察组(n=31)。对照组采用内镜下金属钛夹止血治疗,观察组采用内镜下和谐夹止血治疗,比较两组围术期指标水平、恢复情况、止血效果和术后1周并发症发生率。结果:观察组止血夹使用数量、输血量均少于对照组,止血夹脱落时间长于对照组,差异有统计学意义(P<0.05);两组呕血消失时间、大便潜血转阴时间和住院时间比较,差异均无统计学意义(P>0.05);观察组即时止血率、止血有效率均高于对照组,再出血率低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:消化内镜下和谐夹止血治疗上消化道出血患者可减少止血夹使用数量和输血量,延长止血夹脱落时间,提高即时止血率和止血有效率,降低再出血率,效果优于内镜下金属钛夹止血治疗。Objective:To compare effects of rotary repeated opening and closing soft tissue clip(harmonious clip)and metal titanium clip in treatment of upper gastrointestinal bleeding under digestive endoscopy.Methods:A prospective study was conducted on 61 patients with upper gastrointestinal bleeding admitted to the hospital from January 2019 to November 2021.They were divided into control group(n=30 cases)and observation group(n=31 cases)according to the random number table method.The control group was treated with metal titanium clip hemostasis under digestive endoscopy,while the observation group was treated with harmonious clip hemostasis under digestive endoscopy.The perioperative index levels,the recovery,the hemostatic effects,and the incidence of complications 1 week after the surgery were compared between the two groups.Results:The number of hemostatic clips and the blood transfusion volume in the observation group were less than those in the control group,the dropping time of hemostatic clips was longer than that in the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in vomiting disappearance time,the time for fecal occult blood to turn negative,and the hospitalization time between the two groups(P>0.05).The immediate hemostasis rate and the hemostasis efficiency of the observation group were higher than those of the control group,the rebleeding rate was lower than that of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:The harmonious clip in treatment of upper gastrointestinal bleeding under digestive endoscopy can reduce the number of hemostatic clips and the blood transfusion volume,prolong the dropping time of hemostatic clips,improve the immediate hemostasis rate and the hemostasis efficiency,and reduce the rebleeding rate.Moreover,it is superior to metal titanium clip hemostasi
